Texto de origen - español INSTRUCCIONES DE MANEJO DEL VIVID 7
1. MANEJO BÁSICO DEL EQUIPO:
Encendido del equipo: Pulsar durante un instante para encender el ecógrafo. Para apagar, una vez pulsado el botón aparecerá el menú de apagado.
1. Comienzo de nuevo estudio: pulsando el botón New Patient introducimos los datos del paciente (apellidos, nombre y número de historia.).
2. Selección de sonda: para seleccionar la sonda pulsar el botón Probe y seleccionar la sonda correspondiente. Cuando realizamos cambios en una configuración de imagen y queremos guardar la nueva configuración para una mayor rapidez de manejo del equipo (útil para diferentes tipos de estudios, diferentes tipos de pacientes con mejor o peor transmisión ecográfica o para usuarios con gustos de imagen muy distintos), es suficiente con pulsar el botón Application y seleccionar “preseteos” y grabar la configuración actual o crear una nueva dándole un nombre.
3. Señal ECG: Pulsando el botón Physio, aparecen en los controles de consola los mandos del electrocardiograma, pudiendo modificar su ganancia, posición, velocidad de barrido e incluso desconectarlo.
4. Informe: Pulsando la tecla Report, podemos acceder al informe diagnóstico del paciente, con las medidas y datos demográficos.
5. Worksheet: Tabla de medidas archivadas en la carpeta del paciente.
6. Modos de trabajo: En la parte central de la consola, 2D, Color (Color), Doppler continuo (CW) y pulsado (PW), modo M (M). También podemos hacer aparecer un cursor con la imagen 2D en tamaño normal, utilizando “Cursor”.
7. Control de imagen: La ganancia 2D y la ganancia del modo activo son las dos ruedas giratorias a la derecha de los botones de los modos de trabajo, a la iquierda de estas está el TGC (controles de ganancia en profundidad), y a la derecha de los modos de trabajo la Depth (profundidad), y el Zoom (digital el que se controla con la rueda y de alta resolución el que aparece pulsando la tecla HRZoom).
8. Parámetros de imagen:
Los diferentes ajustes de imagen para cada modo de trabajo se controlan mediante los botones y ruedas de la parte superior derecha de la consola (controles de la consola). La función de estos botones varía según el modo activo, mostrándose en las pantallas iluminadas de la consola.
El botón en forma de estrella en la parte central derecha de la consola controla el llamado “Menú suave”, que también permite el ajuste de diferentes parámetros de imagen.
2D:
Ancho: para estrechar el haz de ultrasonidos.
Ángular: Se encuentra en el menús suave, para angular el haz e incidir lo más perpendicularmente posible en la zona de interés, muy útil en el modo M y en el Doppler CW tricuspídeo.
Cadencia cuadros: modifica el Frame rate o número de imágenes por segundo.
Frec (Frecuencia): menor valor implica mayor capacidad de penetración en tejido; las frecuencias con segundo armónico se indican con la letra H y son las recomendadas en estudios cardiológico.
Color:
Frecuencia de color: presente en el menú suave, una menor frecuencia facilita la penetración del color.
Escala, línea base, varianza (para ver los flujos turbulentos en colores amarillentos o verdosos, en función del mapa de color seleccionado): parámetros presentes en los controles de la consola.
DDP (Persistencia): presente en el menú suave, es recomendable bajarla para ver flujos rápidos.
Prom. lateral y radial: también en el menú suave, conviene ajustar su valor hasta que la apariencia del color nos agrade, valores menores presentarán colores más pixelados.
Prioridad Tisular: dentro del menú suave, indica la prioridad de la imagen 2D frente al color (a menor valor, el color tendrá más prioridad sobre la imagen 2D).
Doppler:
Línea base, Correcc. Angular, controles de la consola (el ángulo Doppler es interesante corregirlo ya que cuando los ángulos son mayores de 60 grados ocasionan graves errores de medida).
Escala del espectro, LPRF (Bajo PRF, proporciona mejor calidad del espectro Doppler, pero limita la escala del Doppler pulsado): presente en los botones de la consola.
Rechazo: umbral de las señales de baja velocidad en el espectro. Presente en el Menú suave.
9. Doppler Tisular Pulsado, para utilizar este modo de trabajo hay que pulsar la tecla TVI en los modos de trabajo
Este modo de trabajo se utiliza para observar el movimiento del endocardio, mediante la aplicación de la muestra de Doppler pulsado (PW) a los puntos del endocardio de interés para observar las ondas Doppler que representan el desplazamiento del tejido.
Consejos útiles de trabajo y optimización de imagen:
- Combinar los parámetros del menú suave “Rango dinámico” y “Compresión” y “Rechazo” hasta alcanzar el nivel de grises y definición de la imagen adecuado. Utilizar “OTA” (presionando el botón de ganancia 2D ) para mayor contraste de bordes (muy útil en la definición de bordes endocárdicos.)
Utilizar valores de “DDP” (persistencia) bajos para Cardiología y prestar mucha atención a la posición del foco, que debe estar a la altura de la estructura de interés o en su defecto, ligeramente por debajo. Utilizar el “Doble Foco” (que aparece en los controles de la consola) para definir mejor los bordes endocárdicos en zonas cercanas al ápex.
- Crear diferentes “preseteos” para pacientes adultos con ventana normal y con mala ventana de exploración, bajando la frecuencia en caso de paciente con mala ventana y obesidad y disminuyendo también la frecuencia del color (en el menú suave) para una mayor penetración del color (especialmente en estudios de venas pulmonares en plano apical.) Trabajar con diferentes “preseteos” reduce el tiempo de exploración al utilizar directamente los parámetros óptimos de imagen para diferentes pacientes o usuarios del ecógrafo.
- Como el ecógrafo trabaja con los datos en formato “Raw Data”, es decir, con los datos “en bruto” de la imagen, podemos modificar todos los parámetros de la recepción tanto en imagen congelada como en imagen archivada. Esto facilita el trabajo puesto que podemos realizar el examen de manera rápida, adquiriendo las imágenes necesarias, y después manipularlas mediante los parámetros de imagen 2D (tales como ganancia, compresión, etc.), color (mapas de color, ganancias, líneas de base) o doppler (línea de base, barrido horizontal, mapas de color doppler). De este modo, podemos mejorar la calidad de las imágenes archivadas y realizar las medidas a posteriori con mejor precisión.
- Al almacenar espectros Doppler, es conveniente guardar el máximo número posible de ondas en la pantalla, para después poder realizar varias medidas diferentes y obtener un promedio. Para ello, basta con reducir el “Barrido horizontal” (control de la consola) antes de guardar la imagen mediante Store. Para la revisión posterior, basta con ampliar el “Barrido horizontal” para poder ver las ondas con precisión.
- Para almacenar cineloops, tanto en 2D como en color, basta con pulsar Store con la imagen en movimiento. El ecógrafo nos presenta el cineloop que va a guardar y es conveniente seleccionar los ciclos de imagen deseados mediante los controles de la consola “Nro. ciclos” o “Marcador izq.” (para el inicio del cineloop) y “Marcador der.” (para el final). Una vez seleccionado el cineloop, pulsar Store de nuevo.
- Para trabajar con la pantalla dividida, es suficiente con pulsar la tecla 1/2/4. Podemos almacenar imágenes en pantalla dividida mediante Store.
- Mediante la tecla Active Mode es posible modificar los parámetros de cualquiera de los modos de trabajo, cuando estamos trabajando con varios modos simultáneamente. Por ejemplo, al realizar un espectro Doppler trabajando con señal 2D y Color, podemos controlar la calidad de la imagen 2D y del color, puesto que con Active Mode pasamos de un menú a otro en los controles de la consola. Además el botón giratorio de ganancia 2D pasará a controlar la ganancia del modo que se encuentre activo.
2. MEDIDAS BÁSICAS:
Para realizar medidas sin que aparezcan en el informe podemos emplear la tecla caliper.
Si deseamos hacer una medida o cálculo predefinidos que se incluyan de manera automática en el informe, es necesario entrar en las distintas carpetas del menú de Measure.
A continuación se detallan las medidas y cálculos más habituales en cada uno de los modos de trabajo.
2D:
Dentro de la carpeta “Dimension”:
SIVd y SIVs: septo interventricular en telediástole y telesístole, respectivamente.
DVId y DVIs: diámetro de ventrículo izquierdo en telediástole y telesístole.
PPVId: pared posterior en telediástole.
Diam Raíz Ao: medida de la raíz aórtica.
Ao Asc.: medida de la aorta ascendente.
Diam AI: medida del diámetro de la aurícula izquierda.
Dentro de la carpeta “Area”:
AVM Planimetría: Planimetría del área valvular mitral.
AVA Planimetría: Planimetría del área valvular aórtica.
Área AI: Área de la aurícula izquierda.
Área AD: Área de la aurícula derecha.
Dentro de la carpeta “Volumen”:
Plano único A4 CH: cálculo de la FE (fracción de eyección) mediante el método de Simpson. Una vez seleccionado este cálculo, es necesario realizar los trazados del VI (ventrículo izquierdo) en telediástole y telesístole del mismo ciclo cardíaco.
Modo M:
Dentro de la carpeta “Genérico”, entrar en estudio de “Estudio de VI”:
SIVd: septo interventricular en telediástole.
DVId: diámetro ventrículo izquierdo en telediástole.
PPVId: pared posterior ventrículo izquierdo en telediástole.
SIVs: septo interventricular en telesístole.
DVIs: diámetro ventrículo izquierdo en telesístole.
PPVIs: pared posterior ventrículo izquierdo en telesístole.
Dentro de la carpeta “Genérico”, entrar en estudio de “AI/Ao”:
Diam Raíz Ao: medida de la raíz aórtica.
Diam AI: medida del diámetro de la aurícula izquierda.
Doppler:
Dentro de la carpeta “Válv. Mitral”:
Vel E/A VM: ratio de velocidades máximas de las ondas E y A de la VM.
VM TMP: tiempo de hemipresión (proporciona automáticamente el área valvular mitral, AVM).
VM Traz: trazado de la onda de la VM para obtener el valor del gradiente medio de presión y el área valvular mitral.
Dur. AVM: Duración de la onda A de la válvula mitral.
Trazado IM: Área de la insuficiencia mitral.
Dentro de la carpeta “Aórtica”:
TSVI Vmax: valor de la velocidad y gradiente máximos del flujo del tracto de salida del ventrículo izquierdo (TSVI).
TSVI Traz: área del flujo del tracto de salida del ventrículo izquierdo.
Vmax VA: valor del gradiente de presión máximo de la Ao.
VA traz.: trazado de la onda de la Ao para obtener el valor del gradiente medio de presión y el área valvular aórtica.
IA TMP: valor del tiempo de hemipresión de la insuficiencia aórtica
TRIV: valor del tiempo de relajación isovolumétrico

IT Vmáx: valor del gradiente de presión máximo de la insuficiencia tricuspídea (IT).

1. BASIC EQUIPMENT OPERATION:
Hold the button down for a moment to turn the ultrasound on. To turn off, press again and the off menu will appear.

2. Choose the probe: to select the probe press the Probe button and select the corresponding probe. When making changes to an image configuration, and we want to save the new configuration thus making management of the equipment faster (useful for different types of studies, different types of patients with better or worse ultrasound transmission, or users with very different image tastes), just press Application and choose “preset” and give it a name.
3. ECG Sign: By pressing Physio, in the first row of the alphanumeric keypad, the on-screen controls for the electrocardiogram will appear, where you can modify the gain, position, sweeping speed, and even disconnect.
4. Report: By pressing the Report key in the first row of the alphanumeric keypad, we can access the diagnostic report of the patient, with the measurements and demographics..
5. Worksheet: Table of actions filed in the patient's folder.
6. Modes of work: In the central part of the console, 2D Color, Continuous Doppler (CW) and pulsing (PW), Mode M (M). We can also make a cursor appear in the 2D image in normal size, utilizing “Cursor”.
7. Image Control: The 2D Gain and the Active Gain Mode are the two turn buttons to the right of the work mode buttons, to the left of these buttons are the TGC (gain depth controls), and to the right of the work modes are Depth (profundity), and Zoom (digital lens that is controlled with the wheel, and high resolution which appears when pressing the HR Zoom key).
8. Parameters of the image:
The different screen adjustments for each work mode are controlled by the buttons on the upper part of the keypad (console controls). The function of these buttons varies according to the active mode, shown in the illuminated screens of the console.
The button in the shape of a star, in the central right part of the control console is called“Soft Menu”, which also permits adjustments of different parameters of the image.
2D:
Width: To straighten the ultrasonic beam.
Angle: To angle the ultrasonic beam making it as perpendicular as possible to the zone of interest, very useful in M and Doppler CW tricuspid modes.
Cadence tables: Modifies the Frame rate or number of frames per second.
Frec (Frequency): Lower value implies greater ability to penetrate tissue; the second harmonic frequencies are denoted by the letter H and are recommended in cardiac studies.
Color:
Color Frequency : present in the soft menu, a lower frequency facilitates the penetration of color.
Scale, base line, variance: (to see turbulent flows in yellowish or greenish colors, in map function in the color selected): parameters present in the console controls.
DDP (Persistence): Present in the soft menu, it is recommended to lower it to see rapid flows.
Prom. lateral y radial: Also in the soft menu, adjust the value until the color you prefer appears, lower value gives more pixilated color).
Tissue Priority: In the soft menu indicate the priority of the 2D image versus color (lower value, the color will have more priority over the 2D image).
Dopler:
Line base, Correc. Angle: Console controls (close attention to the correct Doppler angle is needed because when the angle is greater than 60 degrees it can cause serious measurement errors).
Spectrum scale, LPRF: (Lower PRF, provides a better Doppler spectrum but limits the Doppler pulse scale): present in the console buttons.
Reject: Threshold for low speed signals in the spectrum. Present in the soft Menu.
9. Pulsed Tissue Doppler, to utilize this work mode press the Alt key on the
console, and then choose TDI on the screen controls.
This mode of work is used to observe the movement of the endocardium, through the application of pulsed Doppler sample (PW) to the endocardial points of interest to observe the Doppler waveforms representing the displacement of the tissue.
Useful suggestions for optimization of images:
Combine soft menu parameters "Dynamic Range" and "Compression" and "Reject" until reaching the gray levels and adequate definition of the image. Utilize "OTA" (press the Active Mode button) for higher contrast on the edges (very useful in the definition of endocardial borders.)
Use low values of "DDP" (persistence) for Cardiology and pay close attention to the focus position, which should be at the height of the structure of interest or in it's defect, slightly below. Utilize the “Double Focus" (that appears in the console control) for better definition of endocardial borders in zones close to the apex.
Create different "presets" for adult patients with normal window and poor window exploration, lowering the frequency in a case where the patient is obese with poor window quality and also decreasing the frequency of the color (light menu) for further color penetration (especially in studies of apical pulmonary veins.) Working with different "presets" reduces image scanning time by starting with optimal parameters for different patients or users of ultrasound.
The ultrasound works with the information in "Raw Data" format, in such a way that we can modify all parameters of reception, both in a frozen image as well as an image file. This facilitates the work with the active image because we are working on making the exam faster, acquiring the images necessary, and afterward manipulating them by means of the parameters of the 2D image ( such as gain,compression, etc.), color (color maps, gain, base lines) or Doppler (base line, horizontal scanning and Doppler Color maps). This way, we can improve the quality of the images archived and subsequent measurements are performed with better accuracy.
When storing Doppler spectrums, it is advisable to keep the highest possible number of waves on the screen, so that later we can make various measurements and an average can be obtained. To do this, simply reduce the "horizontal sweep" (control console) before saving the image by using Store. For further review, simply extend the "horizontal sweep" to see the waves with more precision.
In order to store cineloops, as much in 2D as in color, just press Store with the image in movement. The ultrasound presents the cineloop that will be saved and it is advisable to select the image cycles desired by means of the console controls “Number cycles” or “Left Marker” (for the beginning of cineloop) and “Right Marker” (for the end). Once cineloop is selected, press Store again.
To work with a divided screen just press keypad 1/2/4. Images on the divided
screen can be stored using Store.
By using the Active Mode keypad it is possible to modify the parameters of whichever of these modes of work you are using when various modes are being used simultaneously. For example, when using a Doppler spectrum with a 2D signal and Color, we can control the quality of the 2D image, and the color, since with Active Mode we can pass from one menu to another with the console controls. The 2D gain turn knob will control the gain so that is active.
2. BASIC MEASUREMENTS:
To take measurements without them appearing in the report, we can use the caliper key.
If we want a predefined measurement or calculation to be automatically included in the report, it is necessary to to go into different folders in the Measure menu.
Below are the most common measurements and calculations in each of the work modes..
2D:
In the "Dimension" folder:
IVSd and IVSs: interventricular septum in diastolic and systolic, respectively.
DLVd and DLVs: diameter of left ventricle in diastolic and systolic
PWIVd: posterior wall in diastole..
Root Diam Ao: aortic root measurement..
Ao Asc: A measure of the ascending aorta..
Diam AL: measurement of the diameter of the left auricle.
In the "Area" folder:
MVA Planimetry: planimetry mitral valve area.

TI Vmax: value of the maximum pressure gradient of the tricuspid insufficiency (TI).
PRA: value of the estimated pressure of the right auricle (RA), that needs to be entered manually
In the "PISA" folder:
Traced IM: Area of the mitral regurgitation.
IA Trace: Area of the tricuspid regurgitation.
IT Tracing: IM: Area of the tricuspid regurgitation.
Traced IP: Area of the pulmonary regurgitation.

Doppler Espectral: pulsar M/D Cursor (5)– posicionar la línea en la posición deseada – pulsar PW/CW- Doppler (4) - Ganancia (7) – Línea de base (8) – Escala (8) – Velocidad de barrido (8)
Pulsar B (4) para volver a la imagen 2D.
Archivo de imágenes:
Para archivar un cineloop: pulsar Store (11) con la imagen en vivo (sin congelar).
El loop se archiva automáticamente ovisualizar el loop y decidir si es óptimo. Si lo es, pulsar Store
(11) de nuevo. Si no lo es, pulsar el botón Freeze (9) y seguir con el estudio.
Para archivar una imagen fija: pulsar Freeze (9) para detener la imagen, buscar con el trackball para selecionar la imagen deseada y pulsar el botón Store (11).
Medidas: Pulsar Freeze (9)– buscar la imagen deseada con el trackball – pulsar el botón de Medidas (10)
Fin del estudio: Pulsar el botón Patient (2) – Pulsar New Exam – archivar las imágenes si es necesario.

Introduction to the exercises
The 5 Tibetans are a compilation of 21 exercises of Yoga. The practice of these exercises gives again an exceptional vitality and a youth within the body and the spirit. We know that ageing is controlled by the endocrine glands. 2000 years ago, the Tibetan monks also knew this knowledge because they developed a system of exercises which they believed able to renovate these glands by reconditioning the chakras. These exercises are accessible to all, it takes only a little determination and desire to be well. They are rather easy to carry out. They have an effect of youth, of rejuvenation. You may also feel an increase in the libido, your level of joy and your total energy.
Test them and give them a chance.
Start as follows:
The first week, start between 3 and 5 repetitions of each exercise. That will roughly take 5-10 minutes. Each week, add at least two additional repetitions until you reach 21 repetitions of each 5 exercise, which will take you about fifteen to twenty minutes on the whole. It is not necessary or useful to make any more, stop at 21. Breath calmly, deeply and rest between each exercise

The First Tibetan
This exercise reinforces our personal magnetic field. It is really very simple but also very effective. It consists of revolutions, the arms in a cross, the left palm towards the ground and the right towards the sky. The eyes remain open for obvious reasons. The easiest is to swivel on the right foot which remains on the ground and to push with the left foot.
