OPTIMAL INCISIONS – SIZE IS NOT EVERYTHING
One of the recent topics in cataract surgery has been the prospect of reduced incision size. This is because a smaller incision results in less induced astigmatism, faster visual rehabilitation and improved wound security.
Today, for many surgeons the combination of ###l® Torsional ultrasound and micro-coaxial cataract surgery (MCCS) through a 2.2 to 2.4 mm incision represents the optimal surgical approach, since it involves little or no learning curve, less incision leakage, less need for newer instrumentation and excellent visual outcomes.
While noting that the growing availability of premium toric and multifocal IOLs is driving this trend towards ever-smaller incisions, M### W### MD said that there was an important balance to be struck between incision size and wound architecture.
“Smaller incisions are good for reduced astigmatism and also for possibly reducing the risk of age-related macular degeneration induced by cataract surgery, as indicated by some recent studies. However, we do need to maintain perfect wound architecture. Bear in mind that the smaller we go, the higher the stress we are going to place on the incision. Going for a smaller incision that needs to be stretched and that would not be selfsealing would be ultimately counterproductive,” he said.
Dr W### also said that while some surgeons may be comfortable implanting through 2.2 mm incisions using the ###® II/III IOL Delivery System and C or D cartridge (### Inc.) combination, many of his colleagues in France still prefer a larger incision at 2.4 mm.
Wound architecture is vital
R### L### MD said that the new D cartridge has greatly facilitated the transition to 2.2 mm, but also emphasised that size alone was not the only important factor.
“The first important step in successful micro-coaxial cataract surgery is the incision. We must have a perfect incision and I think the perfect incision must allow us to more easily manoeuvre our instruments through the wound. It is vital to have as small amount of leakage as possible. D### A### FRCS, FRCOphth said that while there was an understandable desire to push the limits of small incision cataract surgery, he was not convinced that there was currently any justification for going below 2.0 mm.
“As we get down to 2.0 mm, then we are already into the realm of astigmatically neutral surgery. I don’t see the need to go further below that as we may end up compromising the architecture of the incision because it is too small and it is difficult to move and manipulate instruments. There is also the question of whether it will be feasible to implant high-quality premium lenses through that incision without stretching the wound,” he said.
Dr A### noted, however, that there are particular surgical situations where it may be desirable to use a deliberately larger incision. “There are occasions where the surgeon might want to use a 2.8 mm or 3.0 mm incision to induce an astigmatic change, but a beneficial one rather than a detrimental one. It is about the surgeon always concentrating on what is going to be the best outcome for the individual patient rather than adopting a one size-fits-all approach,” he said.

INCISÕES IDEAIS – O TAMANHO NÃO É TUDO
Um dos tópicos recentes na cirurgia da catarata tem sido a perspectiva de um tamanho de incisão reduzido. Isto porque uma incisão menor tem como resultado menos astigmatismo induzido, uma reabilitação visual mais rápida e uma melhoria na segurança da incisão.
Hoje em dia, para muitos cirurgiões, a combinação do ultra-som ###® Torsional com uma cirurgia de catarata micro-coaxial (MCCS) através de uma incisão de 2,2 a 2,4 mm representa uma abordagem cirúrgica ideal, uma vez que implica pouca ou nenhuma curva de aprendizagem, menos vazamento da incisão, menor necessidade de instrumentação nova e excelentes resultados visuais.
Embora referindo que a crescente disponibilidade de LIOs tóricas e multifocais de primeira qualidade está a provocar a tendência para incisões cada vez mais pequenas, o Dr. M### W### disse que há um equilíbrio importante a atingir entre o tamanho da incisão e a configuração do corte.
"As incisões menores são boas para um astigmatismo reduzido e, também, para potencialmente reduzir o risco de degeneração macular relacionada com a idade induzida pela cirurgia da catarata, como indicam alguns estudos recentes. No entanto, temos de manter uma configuração de corte perfeita. Não nos podemos esquecer que, quanto mais pequena for a incisão, maior é a tensão em que a vamos colocar. Obter uma incisão menor que teria de ser esticada e que não seria auto-selante seria, em última análise, contra-produtivo", referiu.
O Dr. W### também disse que, embora alguns cirurgiões se possam sentir à-vontade a implantar através de incisões de 2,2 mm, utilizando uma combinação dos sistemas de colocação de LIOs ###® II/III ##### e um cartucho C ou D (### Inc.), muitos dos seus colegas em França ainda preferem uma incisão maior, de 2,4 mm.
A configuração da incisão é vital
O Dr. R### L### disse que o novo cartucho D facilitou muito a transição para 2,2 mm, mas também frisou que o tamanho não é o único factor importante.
"O primeiro passo importante para uma cirurgia da catarata micro-coaxial bem sucedida é a incisão. Temos de ter uma incisão perfeita. E eu penso que a incisão perfeita tem de nos permitir manobrar os instrumentos mais facilmente através da abertura. É vital ter o mínimo possível de vazamento". O Dr. D### A### disse que, apesar de haver um compreensível desejo de forçar os limites da cirurgia da catarata com pequena incisão, ele não está convencido que, actualmente, haja qualquer justificação para reduzir a menos de 2,0 mm.
"Quando chegamos aos 2,0 mm, já estamos no reino da cirurgia astigmaticamente neutra. Não vejo necessidade de reduzir mais, pois podemos acabar por comprometer a configuração da incisão, porque é demasiado pequena e é difícil mexer e manipular instrumentos. Também há a questão da viabilidade de implantar lentes premium de elevada qualidade através dessa incisão, sem a alargar", disse ele.
O Dr. ### referiu, no entanto, que há determinadas situações cirúrgicas que podem justificar o uso de uma incisão deliberadamente maior.
"Há ocasiões em que o cirurgião pode querer usar uma incisão de 2,8 mm ou 3,0 mm, para induzir uma alteração astigmática benéfica, em vez de negativa. É necessário que o cirurgião se concentre sempre no melhor resultado para o paciente individual em vez de adoptar uma abordagem 'tamanho único'", disse ele.
