Source text - English Gene therapy is a technique for correcting defective genes responsible for disease development.
Nucleic acid-based molecules (deoxyribonucleic acid, complementary deoxyribonucleic acid, complete genes, ribonucleic acid, and oligonucleotides) are utilized as research tools within the broad borders of gene therapy and the emerging field of molecular medicine. Although most of the nucleic acid-based drugs are in early stages of clinical trials, these classes of compounds have emerged in recent years to yield extremely promising candidates for drug therapy to a wide range of diseases, including cancer, infectious diseases, diabetes, cardiovascular, inflammatory, and neurodegenerative diseases, cystic fibrosis, hemophilia, and other genetic disorders.
Gene therapy may be classified into two types: somatic and germ line gene therapy. There are many ethical, social, and commercial issues raised by the prospects of treating patients using gene therapy.
Deoxyribonucleic acid based therapeutics includes plasmids, oligonucleotides for antisense and antigene applications, deoxyribonucleic acid aptamers, and deoxyribonucleic acidzymes, while ribonucleic acid-based therapeutics includes ribonucleic acid aptamers, ribonucleic acid decoys, antisense ribonucleic acid, ribozymes, small interfering ribonucleic acid, and micro ribonucleic acid.
1 Introduction
Modern drug research aims to discover biologically active molecule(s) that are absolutely specific to the molecular targets responsible for the disease progression.
Moreover, there is strong belief that medicine will soon benefit from the development of new therapeutic technologies to directly target human genes.
Insertion of new genetic material into the cells of an individual with the intention of producing a therapeutic benefit for the patient is human gene therapy while gene therapy is a technique for correcting defective genes responsible for disease development.
Numerous gene therapy strategies are under development, some of which use nucleic acid-based molecules to inhibit gene expression at either the transcriptional or posttranscriptional level and this strategy has potential applications, such as in cardiovascular and inflammatory disorders , neurological disorders, and infectious diseases as well as in organ transplantation.
A number of human diseases are known to be genetic in origin (e.g., Huntington’s chorea and cystic fibrosis), and virtually all diseases, except for some trauma, have a hereditary component Thus, gene therapy represents an opportunity for the treatment of genetic disorders in humans by modifying their cells genetically
Despite the obvious advantages that might be gained from human gene therapy
(i.e., replacing a defective gene with a normal one), there are many ethical, social, and commercial issues surrounding the technology. The outcome of an error in technology might not be observed for many years. Moreover, it is feared that unpredictable and perhaps irreversible side effects occur in treated individuals.
The social implications of such technology include the possibility that patients might suffer from depression as a result of being “genetically altered” or might not be accepted by society in the way that they were before treatment.
The commercial implications of such technology are that the insurance companies and other such institutions also would want to access the available information prior to them granting life insurance policies, etc.
Translation - Arabic تعرف المعالجة الجينية بأنها تقنية تهدف إلى تصحيح الجينات المعطوبة والمسؤولة عن نشأة وتطور الأمراض، الجزيئات المبنية في تركيبها على الحموض النووية كـ (الحمض النووي الريبي منقوص الأوكسجين DNA، الحمض النووي الريبي منقوص الأوكسجين المتممcDNA ، الجينات الكاملة، الحموض النووية الريبيةRNA و قليلات النكليوتيد Oligonucleotide ) تستخدم كأدوات بحثية في الحدود الواسعة لعلم المعالجة الجينية وعلم المعالجة الجزيئية، على الرغم من كون معظم الأدوية المبنية على الحموض النووية لا تزال في المراحل الأولى من الطور البحثي السريري إلا أن هذا الصف من الأدوية قد انبثق بشكل ملحوظ في السنوات الماضية ليغدو مرشحا واعدا يتيح المعالجة الدوائية لطيف واسع من الأمراض من بينها السرطان والتليف الكيسي والناعور وغيرها من الإضطرابات الجينية.
يمكن تقسيم المعالجة الجينية إلى محورين أساسيين:
1- علاج المورثات الجسمية . 2- علاج المورثات التي تنتقل فيها الصفات الوراثية .
ظهرت العديد من العقبات في طريق المعالجة الجينية (كالعقبات أخلاقية – اجتماعية...الخ).
تتضمن المعالجة الجينية المبنية على إستخدام الـ DNA إستخدام البلاسميدات (صانعات البروتين – plasmids) والـ Oligonucleotide في تطبيقات العلاج عن طريق تحقيق حالة antisense والتضاد الجيني، أيضا تشمل استخدام مجموعة DNAzymes (مضاهئات لـ الريبوزومات لكن مع ثباتية بيولوجية أكبر بكثير) وأبتاميرات الـ DNA (Aptamers: قطع من الأحماض النووية مضاعفة الشريط التي تستطيع التفاعل مباشرة مع البروتينات).
تتضمن المعالجة الجينية المبنية على إستخدام الحموض النووية الريبية استخدام RNA aptamers، RNA decoys (الاحماض النووية الريبية المزيفة وهي قطع مصممة لحماية الـ RNA من التبدل عن طريق التنافس مع مواقع ارتباط البروتينات المفعلة للترجمة أو المثبتة لـ الحمض النووي الريبي المرسال mRNA)، والـ antisense، الريبوزومات،عمليات الـ interfering بالحموض النووية الريبية الصغيرة والحموض النووية الميكروية.
1. مقدمة
تهدف الأبحاث الدوائية المعاصرة الحديثة لإكتشاف جزيئات فعالة بيولوجيا تتصف بنوعية فائقة تجاه الجزيئات المستهدفة في العلاج والمسؤولة عن تطور المرض.
أيضا وبالإضافة إلى ذلك هنالك إيمان قوي بأن الأدوية قريبا سوف تستفيد من هذا التطور الذي تشهده الطرق العلاجية لتغدو مستهدفة وبشكل مباشر الجينات البشرية .
يعرف علم معالجة الجينات البشرية بأنه عملية إدخال مواد جينية جديدة إلى خلية دون أخرى بغية الحصول على تأثيرات علاجية بالنسبة للمريض بينما يعرف علم المعالجة الجينية بأنه تقنية تهدف لتصحيح الجينات المصابة المسؤولة عن تطور المرض.
يوجد هنالك كم واسع من إستراتيجيات المعالجة الجينية الي يتم دراستها وتطويرها، منها ما يستخدم الجزيئات المصنعة من الحموض النووية وذلك لتثبيط التعبير الجيني على مستوى النسخ أو في المستويات والمراحل التي تلي النسخ وهذه إستراتيجية ذات آفاق واعدة في مجالات عدة كـ أمراض الأوعية القلبية وفي الإضطرابات الإلتهابية وفي السرطانات والإضطرابات العصبية المنشأ والأمراض المعدية وأيضا في نقل الأعضاء.
يوجد العديد من أمراض البشر المعلومة والمصنفة على أنها من منشأ جيني ( كما في مرض هنتنغتون المعروف أيضا برقص هنتنغتون وأيضا مرض التليف الكيسي )، عموما يمكننا أن نقول وبقوة أنه تقريبا كل الأمراض باستثناء بعض الرضوض تمتلك مكونات مورثية معينة .
تتيح لنا المعالجة الجينية فرصة ذهبية لمعالجة الإضطرابات الجينية لدى البشر عن طريق التعديل بالخلايا البشرية جينيا .
على الرغم من كل هذه الفائدة التي يمكن أن نحصل عليها من علم معالجة الجينات البشرية (كإستبدال جين مصاب بجين طبيعي على سبيل المثال لا الحصر ) إلا أنه هنالك العديد من العوائق ( سواء الأخلاقية منها أو الإجتماعية، أو على المستوى التجاري ) التي تحيط بهذه التقنية .
يمكن أن لا تظهر آثار حدوث خلل ناتج عن هذه الطريقة من المعالجة لسنوات عديدة، أيضا هنالك تخوفات من الآثار الجانبية الغير عكوسة التي تظهر لدى أفراد دون غيرهم .
من ناحية العوائق الإجتماعية التي تقف في وجه هذه التقنية نذكر عبئ شعور الأفراد ومعاناتهم من الكئابة من كونهم "معدلون جينيا" أو من عدم تقبلهم في المجتمع بنفس الطريقة التي كانوا يعاملون بها قبل خضوعهم للمعالجة .
العواقب ذات الطبيعة التجارية لتقنية كهذه تتضمن تلك الناتجة عن شركات التأمين والمؤسسات الأخرى الشبيهة بها، تلك المؤسسات التي سيتطلب دخولها معلومات عن ماضي المرضى تلبية لسياسة التأمين على الحياة وغيرها ...الخ .
