Source text - Polish Badania naukowe pokazują, że personel medyczny w sposób zróżnicowany radzi sobie z intubacją pacjenta, szczególnie w przypadku trudnej intubacji [11,12,13]. Naukowcy wykazują iż nieumiejętna intubacja może spowodować ciężki uraz tkanek miękkich, jak również być przyczyną hipoksycznych uszkodzeń ośrodkowego układu nerwowego [14]. Wówczas pomocą mogą służyć alternatywne metody intubacji, jak urządzenia do intubacji na ślepo bądź wideolaryngoskopy.
Wyniki tego badania pokazują, że ratownicy medyczni z podobną skutecznością wykonują intubację laryngoskopem Macintosh, King Vision oraz ILMA podczas normalnych dróg oddechowych. Jednakże skuteczność intubacji Macintosh znacznie spada w przypadku trudnych dróg oddechowych. badania ujawniły także występowanie statystycznie dłuższego czasu intubacji w przypadku scenariusza trudnych dróg oddechowych w odniesieniu do intubacji urządzeniami Macintosh oraz King Vision. Nie wykazano natomiast aby trudne drogi oddechowe wpływały na wydłużenie czasu w przypadku ślepej intubacji urządzeniem ILMA.
Lin et al. w przypadku intubacji Macintosh w normal airway scenario uzyskali wynik 61.4 ± 4.8, zaś w przypadku difficult airway scenario - 64.3 ± 6.5s. [15]. Zarówno w jednym jak i drugim przypadku czasy intubacji były dłuższe aniżeli w badaniu własnym. Krótszy czas intubacji uzyskał natomiast Chalkeidis et al. - 23.7 ± 5.9s. [16]. Arima et al. uzyskali skuteczność pierwszej próby intubacji laryngoskopem na poziomie 75% zaś skuteczność całkowita wyniosła 100% [17].
Wyniki badan własnych znajdują potwierdzenie w innych badaniach. Murphy et al., skuteczność intubacji w przypadku normalnych i trudnych dróg oddechowych dla urządzenia Macintosh uzyskali odpowiednio na poziomie 100 i 96%, zaś dla King Vision Video Laryngoscope skuteczność ta w obu przypadkach wynosiła 100% [18].
Uzyskana w niniejszym badaniu skuteczność intubacji urządzeniem ILMA wynosząca 97.4% for normal airway scenario and 96.11% for difficult airway scenario, była wyższa aniżeli skuteczność uzyskana przez Michalek et al. – 85.3% [19]. Możliwość wykorzystania ILMA jako skutecznej metody intubacji jest wskazywana również przez Castle et al. [20],

First and foremost it was carried out on a manikin and not on humans, so therefore it is difficult to predict the actual performance of the devices during airway management in the clinical circumstance. Second, is that the phantom could not simulate other clinical factors such as the bleeding.
Translation - English A great number of research presents that there is a great discrepancy, among medical personnel, in the ability to perform successful patient intubation, particularly when it comes to difficult intubation [11,12,13]. The researchers demonstrate, that unskillful intubation may result in severe soft tissue injury, as well as cause hypoxic damage to the central nervous system [14]. Here we can use the support of alternative intubation methods, for instance equipement for blind intubation or video-laryngoscopes.
The results of this research shows that paramedics perform with similar effectivenes using either Macintosh laryngoscope, King Vision or ILMA, in the normal airways scenario. However, Macintosh intubatio efficiency drops drastically for difficult airways. What is more, the study also revealed statistically longer intubation time in difficult airway scenario for Macintosh and King Vision devices. Whereas, difficult airways had not affected the blind intubation time for ILMA device.
Lin et al. Macintosh intubation obtained the score of 61.4 ± 4.8 in the normal airway scenario and 64.3 ± 6.5s. [15] for the difficult airway scenario. In both cases the intubation times were longer than in our study. However, Chalkeidis, during the study, obtained shorter intubation time et al. - 23.7 ± 5.9s. [16]. Arima et al. obtained the results of 75% for first attempt of laryngoscope intubation and the total efficiency of 100% [17].
The results of our research are confirmed in other studies. Murphy et al. obtained the percentage for normal and difficult airways when using Macintosh device amounted respectively at 100 and 96%, and for King Vision Video Laryngoscope percentage in both cases was 100% [18].
Our study showed that, effective intubation with ILMA device amounted at 97.4% in normal airway scenario and at 96.11% for difficult airway, which is higher than results obtained in Michalek study et al. 85.3% [19]. Castle et al. [20] also indicates ILMA as an effective method of intubation.
