Source text - English With the aim of reducing the risk of overtreatment in patients with clinically confined low-risk PCa, 2 conservative management strategies have been proposed: watchful waiting and active surveillance.
Watchful waiting involves regular monitoring of the cancer through tests such as the digital rectal examination and the prostate-specific antigen (PSA) test. Watchful waiting may be used if the cancer has been detected in an early stage or if it is slow growing and confined to a small area of the prostate. It is also used with elderly patients who may not be able to tolerate cancer treatments.
Active surveillance or ‘active monitoring’ includes an active decision not to treat the patient immediately, to follow him with close surveillance and treat at predefined thresholds that classify progression, that is, short PSA doubling time and deteriorating histopathological factors on repeat biopsy.

L'attesa vigile include il regolare monitoraggio del carcinoma con test come l'esplorazione digito rettale e l'esame del sangue con monitoraggio dell'antigene prostatico specifico (PSA). L'attesa vigile può essere utilizzata se il cancro é stato diagnosticato in modo precoce o se ha una crescita lenta ed é circoscritto ad una piccola area della prostata. Viene inoltre utilizzata in caso di pazienti di età avanzata che non potrebbero tollerare trattamenti per il cancro.
La sorveglianza o "monitoraggio attivo" comprende una decisione attiva di non trattare il paziente immediatamente ma di seguirlo con stretta sorveglianza con trattamenti a soglie predefinite che classificano la progressione, ovvero il raddoppiamento del PSA a breve tempo e il deterioramento dei fattori istopatologici nelle biopsie.
