In contrast, among patients with no prior CVD, the benefit to risk ratio for aspirin should be carefully weighed because these patients are at lower absolute CVD risk than patients with known CVD and aspirin increases the risk of bleeding (GI bleeding and, more rarely, hemorrhagic stroke).
