Source text - English Multiple sclerosis is characterised by the presence of scattered, scar-like lesions throughout the brain and spinal cord. Each commences as a patch of inflammatory demyelination, the development of which begins when circulating T-lymphocytes which have, for unknown reasons, become targeted against self myelin and/or oligodendrocyte antigens and subsequently activated; they adhere to and then cross the cerebral endothelium. They then encounter microglia which probably act as secondary antigen-presenting cells; the ensuing lymphocyte/microglial interaction, through mutual activation and release of cytokines and chemokines, produces focal inflammation. Oligodendrocyte and myelin damage follow, though the identity of the cellular and molecular agents responsible remains to be established. Axonal damage also begins to occur at this stage. Limited myelin repair is established, achieved either by surviving or inwardly migrating oligodendrocyte progenitors; it seems likely that areas which fail to repair and/or are repeatedly inflamed experience steadily progressive and wholly irreversible axonal loss. Inflammation and acute demyelination provide the pathophysiological substrate for relapses; chronic demyelination and axonal loss underlie progressive disability. Partial remyelination may contribute to recovery from acute relapse; it is also likely to contribute to axon preservation, and so help reduce the accumulation of permanent neurological dysfunction. Experimental studies provide clear evidence that remyelination can be successfully promoted, but several challenges remain before clinical myelin repair therapies can be explored in neurological practice. This paper reviews the different pathophysiological processes that underlie the clinical presentation of MS and their consequences for therapeutic intervention.
Translation - Italian La sclerosi multipla è caratterizzata dalla presenza di lesioni simili a cicatrici disseminate nel cervello e nel midollo spinale. Ognuna inizia come un’area di demielinizzazione di origine infiammatoria che si sviluppa quando i linfociti T circolanti, per ragioni ancora sconosciute, iniziato a rivolgersi contro gli antigeni della mielina e/o degli oligodendrociti, diventando conseguentemente linfociti T attivati. Questi ultimi aderiscono all’endotelio cerebrale e lo oltrepassano, quindi incontrano le cellule della microglia che agiscono probabilmente da cellule presentanti l’antigene secondarie. La conseguente interazione linfocita-microglia, attraverso la reciproca attivazione e il rilascio di citochine e chemochine, produce un’infiammazione localizzata. Ne consegue il danneggiamento degli oligodendrociti e della mielina, anche se resta da stabilire l’identità degli agenti cellulari e molecolari responsabili. Anche il danno assonale inizia a verificarsi in questo stadio. È stata appurata una limitata riparazione della mielina attribuibile sia alla sopravvivenza, sia alla migrazione verso l’interno, dei progenitori degli oligodendrociti; sembra probabile che le aree in cui il meccanismo riparativo è venuto a mancare, e/o che sono sottoposte a ripetuti fenomeni infiammatori, manifestino una perdita assonale costantemente progressiva e totalmente irreversibile. L’infiammazione e la demielinizzazione acuta costituiscono il substrato fisiopatologico per le recidive, mentre la demielinizzazione cronica e la perdita assonale sono alla base della disabilità progressiva. Una parziale rimielinizzazione può contribuire alla guarigione dalle ricadute di tipo acuto; è anche probabile che essa contribuisca alla conservazione degli assoni, e in questo modo aiuti a ridurre l’accumulo delle disfunzioni neurologiche permanenti. Studi sperimentali hanno fornito una chiara prova che la rimielinizzazione può essere promossa con successo, ma restano da affrontare numerose sfide prima che le terapie di riparazione clinica della mielina possano essere esplorate nella pratica clinica. Questo articolo affronta i differenti processi fisiopatologici alla base della manifestazione clinica della SM e i loro effetti ai fini di un intervento terapeutico.
English to Italian: Irritable Bowel Syndrome

Introduction. Irritable Bowel Syndrome (IBS) is frequently associated with a disequilibrium of the intestinal bacteria. Few studies are available concerning the efficacy and safety of probiotic administration in patients with constipation variant IBS. Recently a symbiotic, constituted by a probiotic, Bifidobacterium longum W11 and by the short chain oligosaccharide prebiotic Fos Actilight, has become available in the clinical practice Aim: to evaluate the efficacy and safety of this symbiotic on clinical symptoms in patients with constipation variant IBS. Methods : 636 patients ( 250 female, 386 male) with diagnosis of constipation-type IBS, according to the Roma II criteria, were enrolled in 43 Centres and received the symbiotic at the dose of 3 gr daily for at least 36 days. A validate questionnaire for symptoms and stool frequency was administered before and after the treatment. Results: According to a visual scale, after treatment a significant increase in frequency was present in the “no symptom” class from 3% to 26.7% for bloating and from 8.4 % to 44.1% for abdominal pain (p

Introduzione. La sindrome dell’intestino irritabile (IBS) è frequentemente associata allo squilibrio della flora batterica intestinale. Sono disponibili pochi studi sull’efficacia e la sicurezza della somministrazione di probiotici in pazienti con la variante di IBS a predominanza di stipsi. Recentemente è diventato disponibile nella pratica clinica un simbiotico costituito da un probiotico, Bifidobacterium longum W11, e dal prebiotico costituito da un oligosaccaride a catena corta Fos Actilight. Obiettivo: valutare l’efficacia e la sicurezza di questo simbiotico sui sintomi clinici di pazienti con la variante di IBS a predominanza di stipsi. Metodi: 636 pazienti (250 femmine, 386 maschi) con diagnosi di IBS del tipo a predominanza di stipsi, secondo i criteri Roma II, sono stati arruolati in 43 Centri e hanno ricevuto il simbiotico al dosaggio di 3 g/die per almeno 36 giorni. Prima e dopo il trattamento è stato somministrato un questionario validato per i sintomi e la frequenza di evacuazione. Risultati: Dopo il trattamento, secondo una scala visiva, nella classe “asintomatica” è stato presente un aumento significativo della frequenza dei sintomi dal 3% al 26,7% per quanto riguarda il gonfiore e dall’8,4% al 44,1% per il dolore addominale (p < 0,0001). Nelle classi più gravi (sintomo moderato-severo), è stata riportata una riduzione significativa della frequenza del sintomo dal 62,9% al 9,6% e dal 38,8 % al 4,1% per il gonfiore e il dolore addominale rispettivamente. È stato anche riportato un aumento significativo nella frequenza di evacuazione, da 2,9 ± 1,6 volte a settimana a 4,1 ± 1,6 volte a settimana. Conclusioni: Nei pazienti con la variante di IBS a predominanza di stipsi il simbiotico può aumentare la frequenza di defecazione e in soggetti con sintomi moderati-severi può anche ridurre il dolore addominale e il gonfiore.

Source text - English Background and Objectives. Heparin might possess anti-thrombotic properties other than anticoagulation. The aim of the present study was to test the effects of a low-molecular weight heparin, parnaparin, on adhesive molecule-mediated platelet-polymorphonuclear (PMN) leukocyte interactions and on PMN function.
Design and Methods. Platelets and PMN were isolated from citrated blood from healthy subjects. Pre-activated platelets incubated with PMN under dynamic conditions formed mixed cell aggregates. In previous experiments PMN were stimulated in vitro by purified Pselectin or formyl-methionyl-leucyl-phenylalanine (fMLP). Dual color flow cytometry was used to detect the formation of platelet-PMN mixed cell aggregates, and PMN activation was tested for by measuring L-selectin shedding, tissue factor expression and PMN degranulation. The effect of parnaparin was compared to that of unfractionated heparin.
Results. Parnaparin, at a concentration of 0.3-0.8 IUaXa/mL, inhibited the formation of mixed cell aggregates (48.8±9.7% of total PMN population) by up to 60% in a concentration- dependent manner, while heparin inhibited aggregation up to 40%. Parnaparin, (0.3-0.8 IUaXa/mL), prevented L-selectin shedding from PMN, which was induced by purified Pselectin (5 g/mL) or fMLP (0.5 mol/L) by 65% and 67%, respectively. Inhibition was independent of incubation time (5-20 min). Parnaparin (0.8 IUaXa/mL) also inhibited tissue factor expression on PMN (% of positive cells), which was induced by P-selectin or fMLP (185±10 and 241±80% of basal value, respectively). Parnaparin protected PMN from degranulation after challenge with either stimulus (>95% inhibition). All the effects of parnaparin were observed with heparin at similar concentrations, although to a lesser extent and were often not significantly different from events in controls.
Interpretations and Conclusions. In conclusion, the process of depolymerization of heparin to obtain low molecular weight parnaparin resulted in an increased, anticoagulant-independent effect on PMN function. Thus, the overall anti-thrombotic properties of parnaparin may be partly due to a leukocyte-mediated anti-inflammatory effect.

Lo scopo del presente studio è stato di testare gli effetti di parnaparina, una eparina a basso peso molecolare, sulle interazioni adesive molecolo-mediate fra piastrine e leucociti polimorfonucleati (PMN) sulla funzione dei PMN.
Disegno e metodi dello studio. Sono stati isolati piastrine e PMN dal sangue citrato di individui sani. Le piastrine preattivate incubate insieme ai PMN in condizioni dinamiche hanno formato aggregati di cellule miste. In alcune sperimentazioni precedenti i PMN sono stati stimolati in vitro con selectina P purificata o formil-metionil-leucil-fenilalanina (fMLP). La flussocitometria a doppio colore è stata utilizzata per scoprire la formazione di aggregati cellulari misti di piastrine e PMN, e l’attivazione dei PMN è stata testata mediante misurazione della liberazione di selectina L, dell’espressione di fattore tissutale e della degranulazione dei PMN. L’effetto di parnaparina è stato confrontato con quello di eparina non frazionata.
Risultati. Parnaparina, a una concentrazione di 0,3-0,8 UIaXa/ml, ha inibito fino al 60% la formazione di aggregati di cellule miste (48,8 ± 9,7% della popolazione totale di PMN) in maniera dipendente dalla concentrazione, mentre eparina ha inibito l’aggregazione fino al 40%. Parnaparina (a una concentrazione di 0,3-0,8 UIaXa/ml), ha prevenuto la liberazione di selectina L dai PMN, che era stata indotta da selectina P purificata (5 µg/ml) o da fMLP (0,5 µmol/L), rispettivamente del 65% e del 67%. L’inibizione è stata indipendente dal tempo di incubazione (5-20 min). Parnaparina (0,8 UIaXa/ml) ha inibito anche l’espressione di fattore tissutale sui PMN (% di cellule positive), che era stata indotta da selectina P o da fMLP (rispettivamente del 185 ± 10% e del 241 ± 80% rispetto al valore al basale). Parnaparina ha protetto i PMN dalla degranulazione dopo la minaccia di altri stimoli (inibizione superiore al 95%). Tutti gli effetti di parnaparina sono stati osservati con eparina a concentrazioni simili, sebbene in grado inferiore, e spesso non sono stati significativamente differenti dai risultati nei controlli.
Interpretazioni e conclusioni. In conclusione, il processo di depolimerizzazione di eparina per ottenere parnaparina a basso peso molecolare ha dato come risultato un aumento dell’effetto sulla funzione dei PMN indipendente da quello anticoagulante. Dunque, le proprietà antitrombotiche complessive di parnaparina possono essere dovute in parte a un effetto antinfiammatorio mediato dai leucociti.
