Il 16.12.___, il 12 e il 15.1. ______ ho visitato il suo paziente.
La sua anamnesi è conosciuta. Egli il 14.12 ____ ha subito una emorragia subarcnoidale, causata da una perdita di sangue da un aneurisma dell’arteria comunicante anteriore sinistra, e il 20.12___ fu operato dalla clinica neurochirurgia di Zurigo. Il paziente da quel momento in poi si è ripreso bene, ora si lamenta di parlare con fatica, occasionalmente ha mal di gola e tosse con la sensazione di avere corpi stranei nella gola. Questi ultimi dolori si manifestano da soltanto due mesi.
Lo stato ORL morfologicamente è competente senza sensibilità, in particolar modo mancano dei cambiamenti patologici oggettivabili nell’ambito di laringe ed ipofaringe.
Almeno radiologicamente non si può trovare nessun punto di appoggio per una sindrome cervicale. La colonna vertebrale cervicale scorre armoniosamente, senza posizione difettosa.
Nel settore destro oto-neurologico giace una debolezza all’udito percepibile a forma di vasca nella gamma alta con perdite d’udito massime fino a 70 ed a 75 dB, a 2000 e 3000 Hz.
In seguito è difficile giudicare se questo difetto uditivo era antecedente (all’operazione) oppure se è connesso all’infezione intracraniale. Una connessione appare piuttosto improbabile, poiché si tratta dell’orecchio della parte opposta dell’aneurisma. All’orecchio sinistro l’udito è fisiologico. Ad un esame al vestibolo risulta sia per i sintomi spontanei quanto nell’esame calorico di stimolo una sinergia ossea isoriflessione di entrambi i labirinti.

On 16/12/___ , 12 and 15/01/___ I examined your patient.
His case study is clear. On 14/12/___ he suffered a subarachnoid hemorrhage, caused by the rupture of the left anterior communicating artery aneurysm. On 20/12/___ he underwent surgery in the clinic of neurosurgery of Zurich. From this time onwards the patient recovered well, but now he complains of speech impairment, occasional sore throat and cough with a foreign-body sensation in the throat. The latter complaints have been appearing only in the last two months.
The ORL condition is efficient in its morphology, without any sensitivity; in particular there are no signs of significant pathological changes in the pharynx and hypopharynx.
At least from a radiological point of view, there is no evidence of cervical syndrome. The cervical vertebral column is undistorted, with no deviation.
Hearing impairment in the right otoneurological side is detected as a bath-tub shape in the high gammas, with hearing losses up to 70 and 75 dB, at 2000 and 3000 Hz.
Furthermore, it is difficult to judge whether this hearing defect was prior ( to surgery) or if it is connected to the intracranial infection. A connection seems somewhat improbable, as it is the ear on the opposite side of the aneurysm. Normal hearing in the left ear. An examination of the vestibule discloses a bone synergy, with symmetric response in both labyrinths, due to spontaneous symptoms and caloric stimulation.

English to Italian: LECTURE ON UNCONSCIOUSNESS
Source text - English Unconsciousness is a condition in which a person is unaware of events around him and he cannot be awakened. He does not wake up when he is called by name or when he is shaken, or when he receives light or strong pain.
Coma differs from this definition in that it is characterized by the fact that pupils do not constrict. Anyway this is considered as one type of unconsciousness.
There are six basic reasons which cause unconsciousness:
1.A disturbance to the nervous system:
a)Fainting (caused by fear, anxiety, sudden change in position, all factors which may result in a change in blood pressure);
b)Seizures (they can be a result of epilepsy or other diseases, such as meningitis, head injury etc.). They last for about two minutes and then patients fall unconscious;
c)Concussion. In this case there is no bleeding, no fracture, there is only soft tissue, that is, the brain may hit the skull in an accident and it gets inflamed. It is a shock that can cause unconsciousness.
2.A fluid problem:
a)Hypolemia (low blood volume); it usually refers to a state of dehydration. In the Third World, for example, it is caused by diarrhoea, which is particularly dangerous for babies;
b)Blood loss, that is, when there is not enough blood in the brain.
3.A problem of what is in the blood or what is missing from it:
a)Overdose (substances which irritate the cells of the brain, ex. drugs, poison, etc.);
b)Metabolites (they are waste of products of chemical reaction). Certain diseases cause the accumulation of metabolites:
Diabetes = in order to produce energy our body uses glucose and fats and sometimes proteins, that we get from food. Our body prefers to use sugar for energy and save fats and proteins to produce hormones and enzymes to re-build cells. So the sugar gives our cells the energy to produce hormones, enzymes and all the functional things that our body needs. Sugar is clean fuel, fat, when burnt, produces ketenes, i.e. metabolites. A diabetic uses fats for energy (if he does not take insulin), because his pancreas cannot secrete insulin. This is a protein that takes sugar from the blood and puts it into the cells: it is a carrier: The problem is that, using so many fats, the diabetic accumulates ketones in his blood, the brain gets poisoned and blacks out. Or it may happen that the diabetic takes more insulin because his need for sugar is increased following to a situation of stress: it is sort of insulin overdose and the patient blacks out because of the rebound effect.
Cirrhosis = this disease is often caused by alcoholism or by a certain kind of hepatitis. The liver cells become replaced by fat cells and gradually the liver stoops functioning: this results in an accumulation of ammonia (metabolite of protein; since the liver is responsible for the metabolism of proteins). Ammonia accumulates in the blood, and this concentration irritates the brain, which blacks out.
Kidney failure = It can result from many diseases, as glomerular nephritis, lupus, certain types of malaria, drug overdoses, etc. Kidneys are responsible for excreting uric acid. If there is too much uric acid in the blood, this may result into uremia (= accumulation of uric acid in the blood). This accumulation irritates the brain cells and causes a coma.
c)hypoglicemia (= too little sugar in the blood); this is something that we all experience when we are hungry: if one continues not to eat, the brain blacks out. The same can happen to diabetic who takes too much insulin or to an alcoholist or a pregnant woman who do not eat enough.
4) Infections:
a)Sepsis (= contamination). Bacteria can harm the organism in two different ways: certain bacteria attack cells, other bacteria, on the contrary, releases a toxin which will get into the blood and travel to the brain, thus causing a sudden decrease in blood pressure. This may provoke a loss of consciousness.
b)Meningitis, which is an infection of the meninges, the membrane that surrounds the brain and the spinal cord. It can be caused by bacteria, funguses, protozoa, viruses, etc. Of course the inflammation of the meninges irritates the brain tissue and may cause unconsciousness.
c)Encephalitis: in this case it is the brain tissue that gets inflamed; it can be caused by the same factors as meningitis.
5) Obstruction of the brain:
a)Stroke. It happens that a piece of clotted blood blocks the circulation of the blood in the brain: the result is a paralysis in the4 opposite part of the body, according to the cerebral lobe that has been affected.
b)Head injury; this causes blood vessels tear and the blood gets out into the brain tissue: this causes an irritation because the brain cells come into direct contact with all substances that blood contains.
c)Neck injury, that is, a broken vertebra; such a fracture might press on the spinal cord and the respiratory control centre, which is in the medulla oblongata; in this way it can interrupt the message to the diaphragm to breathe.
6) Hearth and lung problems:
a)Hearth attack.
b)Respiratory arrest, that is, the lungs stop working for different reasons, ex. enphysema, tuberculosis, etc.
Translation - Italian La perdita di conoscenza é quella condizione in cui una persona é inconsapevole dell'ambiente intorno a sé e non puo' essere risvegliata. Non si risveglia quando é chiamata per nome, né quando viene scossa, né di fronte a luce forte o forti stimoli dolorosi.
Il coma differisce da questa definizione in quanto é caratterizzato dal fatto che le pupille non si contraggono. Questo é comunque considerato come un tipo di perdita di conoscenza.
Ci sono sei motivi principali che possono causare una perdita di conoscenza:
1.Disturbi del sistema nervoso:
a) Svenimento ( causato da paura, ansia, cambiamento repentino di
posizione, tutti quei fattori che possono provocare un cambiamento
di pressione);
b) Attacchi (possono essere il risultato di epilessia o altre malattie come
meningite, lesioni craniche ecc.). Durano circa due minuti e
poi i pazienti perdono conoscenza;
c) Commozione cerebrale. In questo caso non vi é emorragia o frattura,
ma c'é solo del tessuto molle, cioé, il cervello puo' urtare la scatola
cranica in un incidente provocando un'infiammazione. É uno shock
che puo' causare la perdita di conoscenza.
2. Un problema di fluido:
a) Ipolemia (volume ematico basso); di solito si riferisce ad uno stato
di disidratazione. Nel Terzo Mondo, per esempio, é causato da
diarrea, che é particolarmente pericolosa per i neonati;
b) Perdita sanguigna, cioé quando non vi é sufficiente sangue nel
cervello.
3. Un problema in cio' che é presente nel sangue o in cio' che vi é assente:
a) Overdose (sostanze che irritano le cellule cerebrali, per es. droghe,
veleno, ecc.);
b) Metaboliti (sono il prodotto di scarto delle reazioni chimiche).
Alcune malattie causano l'accumulazione di metaboliti:
Diabete = per produrre energia il nostro corpo usa glucosio e grassi ed a volte proteine che ricaviamo dal cibo. Il nostro corpo preferisce usare lo zucchero per l'energia e trattenere i grassi e le proteine per produrre ormoni ed enzimi per ricostruire le cellule. Quindi lo zucchero da energia alle cellule per produrre ormoni, enzimi e tutto il materiale funzionale che il corpo ha bisogno per funzionare. Lo zucchero é un carburante pulito, il grasso, quando bruciato, produce cheteni, cioé metaboliti. Un diabetico usa i grassi per l'energia (se non prende l'insulina) perché il suo pancreas non puo' secernere insulina. Questa é una proteina che prende lo zucchero dal sangue e lo immette nelle cellule: é un trasportatore. Il problema é che, usando cosi' tanti grassi, il diabetico accumula chetoni nel sangue, il cervello si intossica e perde conoscienza. O puo' succedere che il diabetico prenda più insulina perché il suo bisogno di zucchero é aumentato in seguito ad una situazione di stress: é una specie di overdose di insulina ed il paziente perde conoscienza a causa l'effetto rimbalzo.
Cirrosi = Questa malattia é spesso causata dall'alcolismo o da alcuni tipi di epatite. Le cellule del fegato sono sostituite da cellule di grasso ed il
fegato a poco a poco smette di funzionare: questo sfocia in un accumulo di ammoniaca (metabolita di proteina; dato che il fegato é responsabile per il metabolismo delle proteine). L'ammoniaca si accumula nel sangue e questa concentrazione irrita il cervello che perde conoscenza.
Insufficienza renale = Puo' essere il risultato di molte malattie, come la nefrite
glomerulare, il lupus, alcuni tipi di malaria, overdosi di droga, ecc. I reni sono responsabili dell'espulsione dell'acido urico. Se c'é troppo acido urico nel sangue, si puo' arrivare all'uremia (accumulo di acido urico nel sangue). Questo accumulo irrita le cellule cerebrali causando il coma.
Ipoglicemia (= troppo poco zucchero nel sangue); questa é una
condizione che proviamo tutti quando abbiamo fame: se si continua a non mangiare, il cervello perde conoscenza. Lo stesso puo' succedere ad un diabetico che prende troppa insulina o ad un alcolizzato o ad una donna incinta che non mangia abbastanza.
4)Infezioni:
a) Sepsi (= contaminazione). I batteri possono nuocere
all'organismo in due modi differenti: alcuni batteri attaccano le
cellule, altri batteri, invece, sprigionano una tossina che si
introdurrà nel sangue ed arriverà fino al cervello, causando
percio' un'improvvisa diminuzione nella pressione sanguigna.
Questo puo' provocare una perdita di conoscenza.
b) Meningite, che é un'infezione delle meningi, la membrana che
circonda il cervello e la spina dorsale. Puo' essere causata da
batteri, funghi, protozoi, virus, ecc. Ovviamente l'infiammazione
delle meningi irrita il tessuto cerebrale e puo' causare uno stato
di incoscenza.
c) Encefalite: in questo caso é il tessuto cerebrale che si infiamma;
puo' essere causato dagli stessi fattori della meningite.
5)Ostruzione a livello cerebrale:
a) Ictus. Succede che un coaugulo di sangue blocchi la
circolazione del sangue nel cervello:il risultato é una
paralisi nella parte opposta del corpo, a seconda del lobo
cerebrale che é stato colpito.
b) Trauma cranico; questo provoca la rottura di vasi
sanguigni ed il sangue fuoriesce nel tessuto cerebrale:
questo provoca un'irritazione perché le cellule cerebrali
vengono a trovarsi a diretto contatto con tutte le
sostanze contenute nel sangue.
c) Trauma cervicale, cioé, una vertreba spezzata; una tale
frattura puo' esercitare una pressione sulla colonna
vertebrale ed il centro della regolazione respiratoria, che
si trova nella medulla oblongata; impedendo in tal modo
la trasmissione del messaggio di respirazione
al diaframma.
6)Problemi cardio-polmonari:
a) Infarto.
b) Arresto respiratorio, cioé, i polmoni smettono di funzionare per
svariati motivi, per esempio enfisema, tubercolosi, ecc.
