It has been recognized for many years that diabetic patients have a significantly greater incidence and severity of angina, acute myocardial infarction, and congestive heart failure, compared with nondiabetic patients. Although an increased incidence of atherosclerosis in diabetics contributes to these complications, population-based studies have shown that noncoronary factors are also contributory factors.
Abnormalities in myocardial energy metabolism in the diabetic population are probably important contributing factors to this greater mortality. Normal cardiac function is dependent on a constant rate of synthesis of ATP by mitochondrial oxidative phosphorylation and glycolysis. Oxidation of fatty acids is normally responsible for about 60% to 70% of the ATP, with the balance coming from the oxidation of pyruvate derived from glycolisis. This is particularly evident during ischemia, which results in profound disturbances in myocardial substrate utilization, leading to contractile dysfunction.
Diabetic patients, even in the absence of ischemia, have decreased myocardial glucose and lactate uptake and a greater use of free fatty acids and ketone bodies relative to the nondiabetic population.
Accumulation of fatty acids and their toxic intermediaries has been associated with mechanical dysfunction and cell damage in the diabetic heart subjected to ischemia.
Several pharmacological approaches are available to suppress fatty acid oxidation and increase flux through PDH these may directly inhibit the fatty acid pathway or stimulate the glucose pathway. Studies in isolated hearts suggest that such therapies would be of benefit to the diabetic heart during and following myocardial ischemia.
As a result, there is a clear rationale for using metabolic agents to improve contractible function and decrease irreversible damage following myocardial ischemia.

Durante muchos años se ha aceptado que los pacientes diabéticos tienen un índice de aparición y gravedad considerablemente mayor que los pacientes no diabéticos en afecciones como angina de pecho , infarto de miocardio agudo o insuficiencia cardíaca congestiva. Aunque el mayor índice de ateroesclerosis que se da en los pacientes diabéticos contribuye a estas complicaciones , estudios basados en población muestran que factores no coronarios influyen asimismo a este respecto.
Las anomalías que se producen en el metabolismo de la energía miocardia de la población diabética son probablemente importantes factores que contribuyen a esta mayor mortalidad. La función cardíaca normal depende de un nivel constante de síntesis de ATP (o adenosin trifosfato) mediante el proceso de fosforilación oxidativa mitocondrial y glicólisis. La oxidación de los ácidos grasos es responsable normalmente de entre el 60% y el 70% del ATP , teniendo en cuenta que el equilibrio de la oxidación procede del piruvato derivado de la glicólisis. La acumulación de ácidos grasos y de sus intermediarios tóxicos se ha asociado con la disfunción mecánica y el daño celular que se da en el corazón del diabético aquejado de isquemia.
Están disponibles varios enfoques farmacológicos que pretenden suprimir la oxidación de los ácidos grasos e incrementar el flujo a través del PDH ( complejo multienzimático Piruvato Deshidrogenasa). Estos podrían dificultar el camino del ácido graso o estimular el paso de la glucosa. Estudios realizados en corazones aislados sugieren que tales terapias serían beneficiosas para el corazón del diabético durante y después de la isquemia miocárdica.
Como resultado de todo ello puede concluirse que hay un clara base lógica para usar agentes metabólicos con el fin de mejorar la función de contracción y disminuir el daño irreversible que es consecuencia de la isquemia miocárdica.
