I believe the person saying it might have faced some sort of physical altercation that left him with facial injuries. I believe this to be implied in one of the sentences before "ahora ni a mi madre así verla a la cara porque-" [I can't even see my mom like this to her face because]. I didn't get the feeling that this was an idiom of some sort with a hidden or double meaning.
