resident patients might work well for Heimpatienten (2nd link), ventilator-dependent seems to be an etsablished term, as well( 1st link).

Comment: "I like ventilator-dependent. Resident may be misleading, since that could mean a resident in a nursing home, too, for instance. Since this is an outpatient facility, I assume patients on ventilators living at home come in for checkups.
Therefore, "home care" (Melek) wouldn't apply either.

Since I do not know exactly what'Heim' stands for in this context, I have 2 proposals to make:
1) ...home care of ventilator-dependent patients
or
2) ...care of ventilator-dependent patients in nursing homes
I would probably go with the first answer, since 'Heim' and 'home' are both comparatively vague.
And instead of ventilator you can also use 'respirator'; according to Roche both are equally correct.
