My interpretation: the hernial sac was relocated/moved under the oblique muscle (musculus obliquus) - or even sutured to that muscle - in craniolateral position, using a blunt instrument.

"Obliquus internus" seems to be the musculus obliquus internus abdominis (internal oblique muscle of the abdomen).

This is a mere approximation - I'm neither sure about nor satisfied with the "blunt instrument".

I deliberately avoided "dissection" as I'm not really convinced that the term is appropriate here.
