In relation to a 'cocoon' being developed fro premature babies:
Du point de vue orthopédique : les cocons permettent de limiter les plagiocéphalies liées en partie au positionnement dorsal impératif pour la prévention des morts subites du nourrisson. Non regroupé, le schéma corporel se fait en extension et l’enfant est « fort en arrière ». I'm not sure whether it means the infant is 'behind' in his development or whether he develops a strong back!!! As you can see, I need some advice or I could get this badly wrong...

SJLD has pretty much explained this. It doesn’t mean the baby is “behind” but that s/he cannot flexed as term babies can, because of low muscle tone. If you put a prem. baby on their back,s/he tends to splay out like a frog.

"If not supported in flexion, the baby will tend to be extended with a flattened posture" is a suggested translation for the expression rather than an explanation!
The baby has to be nursed in supine, so the issue then becomes 2o pbs such as plagiocephaly, hip subluxation, ...therefore must be helped into a flexed position in supine

... all what I'm not too sure, but this is my interpretation.
The body schema develops in an extended position with an overconcentration on the back.
This is an example of a text where you really need to be able to consult with the author. I suggest you do if this is possible. Otherwise lots of TNs!

I think it does mean the baby's development is concentrated in the back muscles (see the ref I posted). But this term seems to have been invented by the designers/promoters of the cocoon bizzo.
