has been excessively noisy to the irritation of at least one other patient.

what's the meaning of the sentence?

some behaved so noisy that it led another patient to an irritated state

Another patient became a bit furious ;-) because someone was too noisy. ;-)

upsetting

irritation in this context means the patients were getting upset/annoyed/vexed,through the noise being such a nuisance!.
