As in "major disorder" or "major mood disorder" or "major depressive disorder". What is the most appropriate translation of "major"? Some say "erge", but that means severe. Some say "ernstige", but that means serious. One can have a non-severe, non-serious major disorder, after all :-)

According to the English-Afrikaans medical dictionary, "major" stays "major". From experience with numerous Afrikaans psychology texts I also know that the word "major" is used in Afrikaans to distinguish between depressive disorders. Therefore I would use "major".
