Just a couple of thoughts really.
1) I don't see how 64.01.1964 can be a date of birth. Most months don't have that many days!
2) If it is a typo, and what you need is the translation for 'date de naissance', the standard abbreviation is DOB.

Perhaps they have a certificate of good conduct numbering systme accordingly, it may not be bmt like bon service but an offical numbering of any institution such as shipowners or any other insttitution.

The acronimic N.N isn't much used in english any more. I'm not sure that "anon" is really any better, especially in this context.
I'd really need to know what the number is - as someone else said, there aren't 64 days in any month... It certainly isn't a french social security number. They always start with a 1 or a 2 for man or woman and have the departement of birth in the middle. The year of birth isn't shown in full either.
