The following is from a patient's rights booklet. It refers to patients being able to avoid changing doctors when it is not in their best medical interest.
I have translated it, but am not sure that what I have come up with the best translation.
"The medical exemption is designed to protect people with complex medical conditions from having their care interrupted before their treatment is completed or before it is safe to switch to a new provider."

La exención médica tiene como propósito proteger a las personas con condiciones médicas complejas para que no se les interrumpa la atención médica antes de que se complete su tratamiento o antes de que puedan cambiar con seguridad a otro proveedor de servicios de salud.
I'm not a native Spanish speaker, but I understand the passage and you may find my suggestion to be a useful comparison at least.

I guess the problems is that in your doc., the term is used in different ways and you just have to know which one is the best for that application.

I basically agree with Alan, but I think that it refers to people being exempted from having to switch doctors because of their particular medical conditions; so, I would say "por causas médicas". Does it fit the whole context?
P.E. Medical Exemption - If a child is unable to participate in physical education because of injury, illness or a disabling condition, parents are asked to write a note requesting a P.E. exemption
