It is the subject line of a police report "Verbale di notifica di informazione sul diritto di difesa"

I would just use notification (without translating verbale), but maybe there is a better option.

The document is indeed very formal, and at the end of it, it says one copy is provided to the suspect and the original is included in the case file of Tribunal/Prosecutor's office.

It sounds more like a standard advice document given to all suspects, rather than a "report". Is that the case? If so, I would head it something like "Your right to a defence".

It is a report written by Criminal Police Officers and it is served to the suspect. It contains information on legal assistance. I suppose, however, that it could also be used as a record for the court after notification.

When you say "police report", who is this document intended for? Is it a record for the court that the suspect has been notified of their rights, or is it information for the suspect?

Again, US/Can procedure differs from the British Isles, Oz and NZ. So the target readership would be helpful, rather than concocting a neutral translation.

Remember the UK PACE - Police and Criminal Evidence Act 1984 which no one so far has quoted.

If the Aisan in or from a British Commonwealth country, then the BrE option is a likelier bet.
Example sentence:

Note from asker:
Unfortunately the target reader is most likely a native speaker of an Asian language. I am sure it is not for a country where English is the official language.
