From a report on a bicycle damaged as a result of an accident.
Se observa que ****la cadena no está correctamente guiada en el cambio****, está fuera de su recorrido normal a la altura de la roldana intermedia. Con la cadena en esa posición la bici puede rodar, pero el ciclista no puede pedalear, o puede pero la cadena hace mucho ruido y la sensación es como si se fuera a romper todo, no se mueve con suavidad.
I'm especially concerned with the translation of "cambio" - gear system, gear wheel?

James, this really isn't my field of work but here goes:
How Derailers Work
A derailer leads the chain from one sprocket to another, while the chain is moving forward. The front derailer is a simple guide that moves the chain from side to side.
The rear derailer is a bit more complicated, because it also contains the spring and pulleys that adjust the length of the chain as it moves to different-sized sprockets.

I do like Edward's suggestion of "....not properly lined up/positioned...".
