This report provides a picture of the student’s current situation, skills, and needs. All areas of suspected disability were evaluated with conclusions supported by multiple sources of information (including records, observations and interviews). The tests used in this assessment were individually administered, current, reliable, and normed on representative samples. They were appropriate for the student’s age and/or grade level, and they were administered either nonverbally or in the student’s language through an interpreter. Assessment methods were selected for less culturally and linguistically biased methods and in accord with the requirements of the California Education Code. However, scores from the tests should be interpreted with caution given the fact that they are standardized on children who may come from other backgrounds and communities than the student’s. In the professional judgment of the evaluating team members, this assessment is a valid measure of the student’s current level of functioning.

Now let’s look at each of the state measures in more detail.
1. Chronic Absenteeism
The Chronic Absenteeism measure shows how many students were absent for 10 percent or more of the total instructional school days. For example, most schools have 180 instructional days; if a student is absent 18 or more of those days, the student would be considered chronically absent. Students who are chronically absent miss out on important classroom instruction. Capturing this information in the Dashboard allows parents and educators to see if chronic absenteeism is a problem at a school or across a district.
2. Suspension Rate
The Suspension Rate measure shows the percent of students who were suspended anytime during the school year. Students who are suspended from school miss out on important classroom instruction. Capturing this information in the Dashboard allows parents and educators to see if there is a suspension problem at a school or across a district and whether certain student groups are suspended more than others. (Note: A student is counted only once for this measure even if he or she has multiple suspensions throughout the year.)
3. English Learner Progress
Proficiency in the English language is a first step for students to do well in other subject areas, such as reading, writing, mathematics, and science. The English Learner Progress measure looks at the progress that English learners (e.g., students who don’t speak English as their first language) are making toward English proficiency. Each spring, English learners take the English Language Proficiency Assessments for California, or ELPAC, which measures how well they know and understand English. Results from the ELPAC are used to calculate the English Learner Progress measure. (Note: Because only two years of ELPAC data were available in fall 2019, the 2019 Dashboard reported only the percent of students making progress toward English languages proficiency. No performance level, or color, was available.)
4. Graduation Rate
The Graduation Rate measure applies only to schools and districts that serve high school students. For traditional schools, this measure is based on the number of students who graduate with a regular high school diploma in four years or five years. (Note: Students who earn a Special Education Certificate of Completion or a general equivalency diploma are not counted as graduates.) Regardless of whether students go on to college, a high school diploma is the minimum requirement for most entry-level jobs in today’s economy. It represents mastery of foundational skills in mathematics, reading, and writing.
5. College/Career Readiness
The College/Career measure shows how many students graduate from high school better prepared for college or a career. It uses many different measures of college and career preparedness, looking at the courses students completed in high school or the exams they passed. 6. Academic: English Language Arts and Mathematics
The Academic measure shows how well students are meeting grade-level standards in English Language Arts and mathematics. It is based on student performance on the Smarter Balanced Summative Assessments and the California Alternate Assessments, which are taken each spring by students in grades 3–8 and grade 11. Note: If less than 95 percent of all students are not tested, performance on this measure is adjusted downward.

Prior to your initial consult, your records including imaging studies are evaluated by our physicians. All x-rays (including mammograms) done outside our hospital may need to be requested by Radiation Oncology dept.
Your physician will order verification imaging as part of your treatment course. These x-rays are not used as a diagnostic test but used to ensure accurate positioning according to your treatment plan.
QUALITY CHECKS
Each morning before any patients are treated, the machine is checked for performance and quality control. This verifies that the machine is delivering the amount of treatment we need to deliver.
FOLLOW UP
Follow up visits are important for Radiation Oncology so that we may monitor your progress as well as address any side effects you may be experiencing. After your initial follow up visit, generally you will be scheduled at 3-6 month intervals, depending on your situation and how you are doing.

This will be a prospective, randomized and comparative study including 20 patients with myopia or myopia with astigmatism for both eyes. Both eyes of all patients must qualify for custom refractive treatment with both the XX procedure and the YY procedure. Each patient will have their treatment randomized for treatment between eyes, with one eye having treatment with the ZZ system and the fellow eye having treatment with the WW system. Enhancement procedures will not be allowed in this study. Both eyes of enrolled patients must be included in the study. The primary efficacy variables for this study are visual acuity, contrast sensitivity and change in higher-order aberrations. Neither patient nor investigator that will evaluate the postoperative tests will know which equipment was used in each eye.
Subjects will be followed over a 6-month period. A proposed examination schedule is listed in Table 1.
Postop visit schedule includes required study visits only. The investigator is free to choose additional pre or postoperative visit times if appropriate.
