In the US the best general translatiion is "medical invoice," while medical claim form is only submitted through an insurance company. A 'medical invoice' may be issued directly to a patient and paid upon release from medical facility or at the doctor's office.

@franglish In my experience there is very little difference between the two documents in France... I think perhaps a box was ticked and it was on slightly more official-looking paper! My suggestion is of course just a good guess and I'm not 100% confident, but the translation is certainly not "claim form" (which I would expect to be submitted separately or with this document) or the claim itself. It's more like a supporting document to show that you're not claiming money you shouldn't, I believe.

@Marie the only difference in the two bills is that the one for one's health insurance is itemized. The French translation in Switzerland is 'justificatif de remboursement'.

Well the Rechnungskopie is just a copy of the invoice... I've submitted a suggestion but it's not absolutely watertight. France has a similar system - which I've experienced first-hand - so hopefully the answer will help!

Hi Marie It's the name of the described document. With it one gets a 'Rechnungskopie', for the patient to be kept.

disagree Marie Jackson : I'm not sure if this is quite right. If the reimbursement were confirmed, this would be by the authorities, and my impression is that this document is simply the initial document provided by the doctor for the purposes of making a claim.

Reimbursement certificate

Without further context I would suggest this answer. I believe that there is a similar system in France whereby you have to have a specific form filled out by your doctor/medical professional for you to be able to get a reimbursement from social security. A simple invoice is not enough (I speak from experience!).

Thanks for your lively contribution, Marie. As I've explained, in Switzerland the itemized reimbursement receipt is the only document needed to get refunding from one's health insurance. Less complicated than elsewhere, it seems.

Most hospitals and many doctors require direct payment of benefits through an Assignment of Benefits form and will submit the claim for you.
In some instances your doctor may bill you directly, and you will need to submit a claim form to Cigna HealthCare for reimbursment of a percentage of the covered expenses after you've met your annual deductible.

Thank you, Ingrid. In Switzerland, we receive an itemized bill from the doctor/hospital specifically for our health insurer. All we need do is send it in to be reimbursed. So in fact it is simply a reimbursement receipt. I'll stick to that translation.
