El Test de Dislexia Bangor es un instrumento de aplicación individual pensado para sujetos a partir de 7 años, puesto que si el escolar es menor de esta edad algunos ítems le serán, casi seguro, demasiado difíciles; e incluso en el caso de sujetos de 7 y 8 años se deberá ser cauteloso al establecer conclusiones. El test no se debería aplicar a sujetos con una capacidad de aprendizaje inferior a lo normal; ello no significa que tales sujetos no puedan considerarse disléxicos, pero posiblemente sus necesidades educativas sean diferentes a las de los “clásicos” disléxicos descritos en la literatura.
El test consta de 10 ítems que se puntúan con un sistema de indicadores positivos o negativos. Esto es, se pone un signo si en una determinada tarea el sujeto fracasa (o da muestra de dislexia), un 0 si satisface los criterios menos exigentes de este trastorno y un signo − si tiene éxito en la tarea. A continuación se describen los diez ítems del test y la fundamentación teórica de cada uno de ellos:
Izquierda—derecha (partes del cuerpo). Se trata de una serie de preguntas cada vez más complejas para estudiar el reconocimiento de la orientación, de modo que las nociones de derecha e izquierda interactúan con el esquema corporal, como Muéstrame tu mano derecha, Toca tu oreja derecha con tu mano izquierda o Señala mi ojo izquierdo con tu mano derecha. La dificultad que experimentan los disléxicos en este ítem vendría explicada por la inconsistencia con que utilizamos las etiquetas ‘izquierda’ y ‘derecha’, ya que dependen del lugar donde estemos situados. Esta inconsistencia, que ocasiona problemas a todos los sujetos, se añade a la dificultad en el etiquetado verbal que tienen los disléxicos.
Repetición de palabras polisílabas. En este ítem se pide al sujeto que repita las palabras ‘preliminar’, ‘filosófico’, ‘contemporáneo’, ‘anémona’ y ‘estadístico’. Dado que las lenguas románicas peninsulares (castellano, gallego y catalán) tienen más palabras polisílabas que el inglés, decidimos incluir las palabras ‘ensimismamiento’, ‘independientemente’ e ‘inconmensurabilidad’ para aumentar el grado de dificultad de este subtest. En la versión catalana hemos utilizado la palabra ‘enllumenament’ por la inexistente ‘ensimismamiento’. Los criterios de selección de estos términos fueron similares a los seguidos por Miles. Según este autor, la complejidad en la pronunciación de palabras polisílabas con fonemas repetidos y/o sonidos parecidos interfiere en la recuperación de tales palabras, pudiendo alterar el orden correcto de las sílabas.
Restas y Tabla. En el ítem de las restas se trata de comprobar si el sujeto es capaz de realizar operaciones básicas de dificultad creciente, como 6-3, 19-7, 54-9…, de recordar la dirección en que se nombran los números y de obtener la respuesta correcta sin utilizar estrategias compensatorias. Por su parte, en el subtest de la tabla se pregunta al sujeto la tabla de multiplicar de tres números, que normalmente deberán ser la del seis, la del siete y la del ocho. En el caso de niños de 7 y 8 años sólo se le pide la tabla del cuatro.
Para Miles, la dificultad en el etiquetado verbal también afecta a algunos aspectos de las matemáticas. Según este autor, los disléxicos tienen un repertorio menor de ‘hechos numéricos’ o combinaciones numéricas básicas, entendiendo como ‘hechos numéricos’ la capacidad para dar una respuesta automática a una determinada operación. Así, muchos sujetos sin dificultades en el aprendizaje pueden responder correctamente y con rapidez a problemas como 8 x 7 = 56 ó 44 – 7 = 37; pero cuando se les pregunta cuánto es 27 x 23 ó 491 – 327, necesitan un tiempo mayor para dar el resultado, porque las respuestas a esos problemas no están almacenadas en su memoria a largo plazo. Los disléxicos, por su parte, dominan menos combinaciones básicas y necesitan reforzar más cada asociación problema-respuesta para que esta sea automática, ya que el dominio de ‘hechos numéricos’ se fundamenta en la asociación de datos y el aprendizaje significativo de la aritmética.
Meses hacia delante, Meses hacia atrás, Dígitos hacia delante y Dígitos hacia atrás. En los ítems de los meses se pide al sujeto que diga los meses del año en orden directo e inverso. Por su parte, los ítems de dígitos son pruebas tradicionales de algunos de los test más utilizados, como el WISC, en los que el sujeto tiene que repetir una serie de dígitos en un orden determinado. El bajo rendimiento de los disléxicos en estos ítems vendría explicado también por la dificultad en el etiquetado verbal, ya que este problema se agrava cuando el sujeto tiene que asociar elementos de una serie que no están relacionados lógicamente, como el aprendizaje de los meses del año o la repetición de dígitos. El tiempo necesario para encontrar la palabra adecuada, reduce el buen funcionamiento de la memoria inmediata. Por tanto, el éxito de sus respuestas estará en función de la longitud de las series y las oportunidades de aprendizaje. Cuando los ítems tienen que decirse en orden inverso, se añade una dificultad más a la tarea inicial, complicándose más la prueba.
Confusión b—d. En este ítem se trata de averiguar si hay evidencia de que actualmente el sujeto confunde ‘b’ y ‘d’ o las confundió más allá de los 8 años. Las confusiones entre b y d (‘reversals’, en general), comúnmente asociadas a la dislexia, vendrían causadas no por un problema visual, sino por una dificultad para distinguir dos etiquetas que tienen un parecido fonológico (Outón, 2000).
Incidencia familiar. Miles sostiene que la dislexia viene determinada por un factor genético. Por ello, considera un dato significativo para el diagnóstico conocer si hay familiares afectados o con dificultades similares.
Translation - English Instrument
The Bangor Dyslexia Test is an individually administered instrument designed for subjects aged over 7, since learners younger than 7 might otherwise find some test items too difficult. Even in the case of 7- and 8-year-olds caution should be exercised before drawing conclusions. The test should not be administered to learners with a lower-than-average learning ability. This exclusion does not mean that such learners cannot be considered dyslexic but that their educational needs might be different to those of the “classic” dyslexics described in medical literature.
The test contains 10 items, which are scored through a system of positive and negative indicators. In other words, a plus ( ) sign is given if the subject fails (or shows dyslexia) in an item; a nought (0), if the subject fulfils the least demanding criteria for this condition, and a minus (–) sign if he/she successfully completes the task. Below is a description of the 10 test items as well as their theoretical foundation:
Left-right (body parts). It is a series of questions of increasing complexity to study the subject’s sense of direction, so that the notions of left and right interact with their body scheme, such as Muéstrame tu mano derecha (show me your right hand), Toca tu oreja derecha con tu mano izquierda (touch your right ear with your left hand) or Señala mi ojo izquierdo con tu mano derecha (point to my left eye with your right hand). The difficulty for dyslexics in this item is explained by the inconsistency with which we use the labels ‘left’ and ‘right’ ¬– since their meaning depends on the place we are. This inconsistency, which causes problems to all subjects, adds to the dyslexics’ difficulty for verbal labelling.
Repeating polysyllabic words. In this item the subject is asked to repeat the words preliminar, filosófico, contemporáneo, anémona and estadístico. Since the Iberian Romance languages (Spanish, Galician and Catalan) have more polysyllabic words than English, we decided to include the words ensimismamiento, independientemente, and inconmensurabilidad in order to raise the level of difficulty of this subtest. In the Catalan version, the word enllumenament has been used because ensimismamiento does not exist. The selection criteria for these terms were similar to those followed by Miles. According to him, the pronunciation complexity of polysyllabic words with repeated phonemes and/or similar sounds interferes with word retrieval, and as a result the correct order of the syllables could be altered.
Subtraction and Tables. The subtraction item tries to check whether the subject is able to perform basic operations of growing difficulty, such as 6 – 3, 19 – 7, 54 – 9, to remember the order in which numbers are read out, and to obtain the right answer without using compensatory strategies. In the tables subtest, on the other hand, the subject is asked the multiplication table of three numbers, normally those of 6, 7 and 8. Only the table of 4 is asked in the case of 7- and 8-year-olds.
For Miles, the difficulty in verbal labelling also affects some aspects of mathematics. According to him, dyslexics have a smaller repertoire of “number facts”, or basic number combinations. Number facts are understood as the capacity to give an automatic answer to a certain operation. Therefore, many subjects without learning difficulties can successfully and quickly solve problems such as 8 x 7 = 56, or 44 – 7 = 37, but when asked about 27 x 23 or 491 – 327, they need more time to provide an answer, because the answers to these problems are not stored in their long-term memory. Dyslexics master fewer basic combinations and need to reinforce more every problem–answer association so that it becomes automatic, because the mastery of “number facts” is based on the association of data and the meaningful learning of arithmetic.
Months forward, Months reversed, Digits forward, and Digits reversed. In the months items, the subject is asked to say the months of the year in normal and reversed order. As the digits items are concerned, they are traditional tasks in some of the more widely used tests, such as WISC, in which the subject has to repeat a series of digits in a certain order. The poor performance of dyslexics in these items might also be explained by their difficulty in verbal labelling, since this problem is compounded when the subject has to associate a set of elements that are not logically related, such as in learning the months of the year or in digit repetition. The time needed to find the right word reduces the performance of immediate memory. The success in the answers depends, therefore, on the length of the series and the opportunities to learn. When the items have to be said in reverse order, an extra difficulty is added to the original task, and it becomes more complicated.
B-d confusion. In this item, we try to find out if there is evidence that the subject currently confuses b and d, or that he/she confused them after the age of 8. B-d confusion (and letter reversals, in general) is commonly associated with dyslexia, and it might not be caused by a visual problem but by a difficulty to distinguish two phonologically similar labels (Outón, 2000).
Familial incidence. Miles contends that dyslexia is due to a genetic factor. He therefore believes that knowing if relatives are also affected or experience similar difficulties is important for diagnosis.
