The most common cause of dementia, this is a progressive degenerative condition in which plaques cause damage to the brain.
Alzheimer's disease is rare before the age of 60, but increasingly common thereafter. Most cases occur without an identifiable cause. Mutations in several genes are associated with this disorder, however, and the genetic component is especially strong in the relatively rare cases of early onset disease (symptoms occurring before 60). In late-onset Alzheimer's disease, mutations in genes responsible for the production of a blood protein called apolipoprotein E are implicated. These genes result in a protein (beta amyloid) being deposited in the brain as plaques, which leads to the death of neurons. Alzheimer's disease is also associated with reduced levels in the brain of the neurotransmitter acetylcholine. Additionally, it is thought that the disruption of the mechanism that controls the inflow of calcium ions into neurons may be involved, leading to excessive calcium in the neurons, which prevents them from receiving impulses from other brain neurons.
Treatment
Treatment for this disorder is aimed at slowing down the degeneration, but it does not completely halt decline, and eventually complete nursing care is needed. Acetylcholinesterase inhibitors may slow progress of Alzheimer's disease in the early and middle stages, and memantine in the later stages.
Protein filaments
Alzheimer's disease is often associated with the formation of tangled masses of protein filaments, which may develop to form plaques.
Stages of Alzheimer's disease
The symptoms and progression of Alzheimer's disease vary from person to person. However, the symptoms become increasingly severe as the disease progresses and larger areas of the brain are damaged, although in some cases there may be periods in which the person seems to improve. Generally, there are three broad stages in the development of Alzheimer's disease.

Première cause de démence, cette maladie neurodégénérative progressive entraîne une altération du cerveau provoquée par la formation de plaques. Rare avant 60 ans, cette maladie est de plus en plus fréquente au-delà. Il n’existe aucune cause identifiable dans la plupart des cas mais des mutations génétiques, qui jouent un rôle conséquent dans les cas relativement rares d’apparition précoce (premiers symptômes avant 60 ans), sont toutefois associées à cette démence. Dans les cas d’apparition tardive, la mutation des gènes responsables de la fabrication d’apolipoprotéine E (protéine présente dans le sang) est en cause. Ces gènes entraînent le dépôt d'une protéine (appelée bêta-amyloïde) sous forme de plaques dans le cerveau, ce qui entraîne la mort des neurones. La maladie d'Alzheimer est également liée à une diminution du taux d’acétylcholine (neurotransmetteur) dans le cerveau. En outre, un excès de calcium dans les neurones bloque l’échange d’informations : la perturbation du mécanisme de régulation de l'afflux d'ions calcium dans les neurones pourrait donc jouer un rôle.
Les traitements
Ils visent à ralentir la dégénérescence des neurones mais ne permettent pas de l'arrêter complètement : tôt ou tard, des soins médicaux complets finissent par s’imposer. La progression de la maladie peut être ralentie par les inhibiteurs de l'acétylcholinestérase aux stades précoces et intermédiaires, et par la mémantine aux stades plus avancés.
Les filaments de protéines
La maladie s’accompagne souvent d’une formation d’enchevêtrements de filaments de protéines (appelée dégénérescence neurofibrillaire) qui finissent par former des plaques.
Les différents stades de la maladie d’Alzheimer
Les symptômes de la maladie ainsi que son évolution varient d'un patient à l'autre. Dans certains cas, la personne atteinte semble aller mieux pendant un temps, mais les symptômes s’aggravent à mesure que la maladie progresse et que la surface touchée dans le cerveau augmente. Trois principaux stades de la maladie ont été définis.
