Source text - English Mutagenic Effects: This product contains beta-(3,4-Epoxycyclohexyl) ethyltrimethoxysilane. This material has been shown to exert activity consistent with mutagenic effects in the following invitro tests: Ames bacterial assay, sister chromatid exchange test in mouse lymphoma cells, unscheduled DNA synthesis in human WI-38 cells, and a forward gene mutation assay in mouse lymphoma cells. Also, this material was shown to induce a low incidence of squamous cell carcinomas in a study involving the chronic recurrent application of the material on the skin of mice. A developmental toxicity study in rats and rabbits showed minimal fetotoxicity at maternally toxic doses but no evidence for embryotoxicity or teratogenicity.

The following document explains the way to file an appeal with the Claims Office. Please note that only one process may apply to your specific situation.
APPEAL TO CLAIMS ADMINISTRATOR
You may file an appeal with the Claims Administrator if:
• You are dissatisfied with the allowed benefits stated on your NOS letter.
• You disagree with the Claims Office's interpretation of the medical issues in your claim.
Your appeal will be limited to the specific benefit status contained in the Notification of Status you are appealing.
Appeal to the Claims Administrator Process
1. Submit a letter entitled ‘Appeal to Claims Administrator’, outlining the reasons for the Appeal.
2. The Claims Administrator will conduct a full review. The Claims Administrator may request additional submissions from you or your physician in deciding the appeal.
3. The Claims Administrator will issue a determination in writing.
APPEAL TO APPELLATE JUDGE
If you are not satisfied with the result of your appeal to the Claims Administrator, you may file an appeal to the Appeals Judge. You may not appeal to the Appeals Judge until you receive a determination on your appeal to the Claims Administrator.
Appeal to the Appeals Judge Process
1. Submit a written statement outlining your position and the reason why you believe the Claims Administrator’s decision is in error.
2. The Claims Office will submit your file and your written statement to the Appeals Judge.
3. The Judge will review your file.
4. The Appeals Judge will issue an order and the decision of the Appeals Judge is Final and Binding.

Vous ne pouvez faire appel auprès du Juge d’appel avant d’avoir reçu une décision de la part de l’Administrateur des plaintes portant sur votre appel.
Processus d’appel auprès du Juge d’appel
1. Soumettre une déclaration écrite décrivant votre prise de position et la raison pour laquelle vous pensez que l’Administrateur des plaintes a tort.
2. Le Bureau des revendications soumettra votre dossier et déclaration écrite au Juge d’appel.
