Paciente de 11 años, con antecedentes de VIH connatal, diagnosticada a los 6 años en etapa N3 (categoría clínica N corresponde al niño infectado asintomático y etapa 3 corres-ponde a inmunodepresión severa). La terapia antirretroviral altamente activa fue iniciada 6 meses más tarde. Presentó baja adherencia al tratamiento, y a los 4 años de iniciado este se confirmó una carga viral elevada. Se realizó genotipificación viral la cual mostró resistencia a los tres antivirales usados. Se decidió cambio de terapia a Kaletra® (lopinavir 80/ritonavir 20) (3,5 ml cada 12 h), abacavir (300 mg cada 12 h) y TDF (300 mg al día), presentado buena adherencia a esta terapia y descenso de cargas virales. Después de 18 meses de iniciada la terapia, presentó pérdida de fuerza y dolor de extremidades inferiores, incapacidad funcional, dificultad en la deglución y pérdida de peso. A su ingreso presentaba un IMC 14,5 el cual está en el percentil 10 (z -1,9) del correspondiente a su edad y una talla/ edad z -1,7 DS. Los estudios de laboratorio mostraron hipofosfatemia, fosfatasas alcalinas elevadas, acidosis metabólica, relación calcio/creatinina elevada, una reabsorción tubular de fosfato disminuida, glucosuria y proteinuria (tabla 1). La resonancia nuclear magnética cerebral y la ultrasonografía renal fueron normales. La densitometría ósea mostró una disminución significativa de la densidad mineral ósea.
Translation - English Clinical Case
This is an 11-year-old patient with a history of connatal HIV, who was diagnosed with stage N3 at the age of 6 (clinical category N is for an infected, asymptomatic child, and stage 3 is for severe immunodepression). Highly active antiretroviral therapy began 6 months later. Her treatment adherence was poor, and 4 years later an elevated viral load was confirmed. Viral genotyping revealed resistance to the three antivirals used. She was changed to Kaletra® (lopinavir 80/ritonavir 20) (3.5 ml every 12 h), abacavir (300 mg every 12 h), and TDF (300 mg daily). Adherence to this regimen was good, and her viral load decreased. Eighteen months after beginning this treatment, she had loss of strength and pain in the lower extremities, functional disability, difficulty swallowing, and weight loss. Upon admission, her BMI was 14.5, which was in the 10th percentile (z -1.9) for her age, and she had a height/age ratio of z -1.7 DS.
Laboratory studies showed hypophosphatemia, elevated alkaline phosphatase, metabolic acidosis, an elevated calcium/creatinine ratio, reduced tubular absorption of phosphate, glycosuria, and proteinuria (table 1). A nuclear magnetic resonance of the brain and renal ultrasound were normal. A bone density test showed significant bone mineral density loss.
