Operation:
Tenolyse Daumenbeugesehne links mit Spaltung des A1-Ringbands, Einkerben des oblique Ringbands und partieller Synovialecktomie der FPL-Sehne
Arthrotomie linkes Daumengrundgelenks, partielle Synovialektomie, raffende Kapsel-Bandplastik (ulnares und radials Kollateralband), Denervierung Daumengrundgelenk links
Indikation:
Schmerzhafte Bewegungs- und Belastungseinschränkung Daumen links
Technik:
In Rücken-Armtischschalgerung erfolgt zunächst die Hautdesinfektion und die sterile Abdeckung in typischer Weise.
Anlage einer OA-Blutleere mit 280 mmHg.
Zunächst Versorgung der Tendovaginitis stenosans der Daumenbeugesehne:
Hautinzision im Verlauf der proximalen Beugefalte über dem Daumentrahl, Blutstillung, stumpfes Vorpräparieren auf das A1-Ringband unter Schonung der begleitenden Gefäß-Nervenbündel. Das Subkutangewebe zeigt sich deutlich entzündlich verändert, die Beugesehnenscheide proximal des A1-Ringbandes aufgetrieben, das A1-Ringbad schnürt die Beugesehne deutlich ein. Spalten des A1-Ringbandes unter Schonung der Beugesehne und Resektion eines 2mm breiten Streifens des A1-Ringbands. Nachdem die Beugesehne sich weiterhin eingeengt zeigen, wird auch das oblique Ringband im Sinne eines sog. Ventings proximal inzidiert. Nun Hervorholen der FPL-Sehne vor den Situs. Die Beugesehne zeigt einen deutlichen Kalibersprung im ehemaligen Bereich des A1-Ringbandes, es besteht eine synovialitische Verklebung, weswegen eine Tenolyse sowie eine partielle Synovialektomie erfolgt.
Abschließend Kontrolle des Sehnengleitens bei passiver Bewegung. Die Beugesehne zeigt sich nicht mehr eingeengt und gleitet gut, es besteht kein Bow-String-Effekt.
Spülung, Kontrolle auf Bluttrockenheit, Hautnaht mittels Intrakutannaht.

Indication:
Painful movement and limited ability to bear weight in the left thumb
Technique:
In the supine position with arm table support, the skin is first disinfected and sterilely draped in the usual manner.
Upper arm tourniquet is applied to 280 mmHg.
First, treatment of the tendovaginitis stenosans of the thumb flexor tendon:
Incision along the proximal flexor crease above the thumb metacarpal, hemostasis, blunt incision of the A1 annular ligament while protecting the neurovascular bundle. The subcutaneous tissue shows marked inflammatory changes. The flexor tendon sheath proximal to the A1 annular ligament is raised. The A1 annular ligament clearly constricts the flexor tendon. Splitting of the A1 annular ligament while protecting the flexor tendon and resection of a 2 mm wide striation of the A1 annular ligament. Because the flexor tendon still appears constricted, the oblique annular ligament is also incised proximally in a venting fashion. Now, the FPL tendon is brought out of its original position. The flexor tendon shows a marked sudden change in diameter in the former area of the A1 annular ligament. There is a synovitic adhesion, for which a tenolysis as well as a partial synovectomy are carried out.
Finally, inspection of tendon sliding during passive motion. The flexor tendon does not appear constricted anymore and slides well. There is no bowstring effect.
Irrigation, inspection of bleeding cessation, suturing using intracutaneous suture.

Glutenintoleranz oder Zöliakie beruht auf einer lebenslangen Unverträglichkeit des Organismus gegenüber dem Klebereiweiß Gluten. Diese chronische Erkrankung der Dünndarmschleimhaut kann sich im Kindesalter manifestieren, aber auch erst im Erwachsenenalter auftreten. Dabei kann das Essen von glutenhaltigen Nahrungsmitteln zu Symptomen wie Bauchschmerzen, Durchfall, Müdigkeit und Gewichtsverlust führen.
Thaizeit: Warum sind Asiaten in der Regel nicht von Zöliakie betroffen?
Zöliakie ist eine Autoimmunerkrankung, die vor allem genetisch bedingt ist. Sie tritt am häufigsten bei kaukasischen Bevölkerungsgruppen in Europa, Amerika oder Australien auf.

Thaizeit: Which food allergies occur most predominantly in the Thai people?
Thais don’t have any specific food allergies. Nevertheless, we have been able to observe a rise in allergic reactions to fish, seafood, and peanuts. In earlier times, we also could document more cases of allergies to cow’s milk in infants.
Thaizeit: What facilities do you offer to those affected by celiac sprue at the Bumrungrad Hospital?
First of all, celiac sprue is difficult to diagnose. The doctor must be well-versed in the disease and have the necessary means to make the diagnosis. The Bumrungrad Hospital is the only clinic in Thailand that has its own diagnostic tools at its disposal. Here we make an accurate and quick diagnosis with specific blood tests. Furthermore, we counsel the patients and support them in coming to terms with the disease.
