Global efforts to reduce infectious disease rates must have a greater focus on older children and adolescents after a shift in disease burden onto this demographic, according to a new study.
The research, led by Murdoch Children's Research Institute and the Institute for Health Metrics and Evaluation, has found that infectious disease control has largely focused on children aged under five, with scarce attention on young people between five and 24 years old.
Published in The Lancet, the study found three million children and adolescents die from infectious diseases every year, equivalent to one death every 10 seconds. It looked at data across 204 countries between 1990 and 2019 from birth to 24 years of age.
Diarrhea, pneumonia and malaria account for two-thirds of infectious diseases and death among children and adolescents. HIV and tuberculosis were the leading causes among older adolescents.
The shift in infectious disease burden from young children to older children and adolescents, was largely driven by the considerable infection control efforts targeting children under five years in low-income countries and slower progress made across the older age groups. In 1990, 85 percent of the infectious disease burden was among children younger than five years, however by 2019 this decreased to 75 percent.
More than half of the deaths among children and adolescents in low to middle-income countries were caused by infectious diseases, compared with 6 percent in high-income countries. India, Nigeria and Pakistan carried the greatest disease burden.
In Australia, the infectious diseases that cause the greatest burden from birth to 24 years are upper respiratory tract infections, such as sinusitis and tonsilitis, and infectious skin conditions like shingles and cellulitis. The leading causes of disease burden in high-income countries are associated with conditions that led to disability, highlighting a need to look beyond just death rates to focus more on the impact of a disease on a person's life.
Murdoch Children's Dr. Jessica Kerr said the policy focus must be widened to include older children and adolescents and more action taken to prevent tuberculosis and HIV.
"Most deaths from infectious diseases are in children younger than five years, so while reducing this has been a historical achievement, it should not be our only focus," she said.
"The significant reduction has driven a shift in disease burden from young children to older children and adolescents, reinforced by the almost exclusive focus on younger children by infectious disease control programs. There is currently an unmet health need among older children and adolescents and we need the global community to push for policy change and funding to address this."
Murdoch Children's Professor Peter Azzopardi said the research would have important implications for global policy, financing, resource allocation and health systems, especially in light of recent health crises.
"The COVID-19 pandemic and recent epidemics of Zika virus, Ebola and severe acute respiratory syndrome underscore the urgent need to take stock of infectious disease control," he said. Some of these emergent diseases have affected adolescents more than younger children, challenging the almost exclusive focus on younger children within existing infectious disease control.
"The findings highlight the need for health systems, particularly in low-income countries, to continue to build capacity and scale up trusted interventions like immunization programs. But there must also be investment in broader approaches that address social barriers such as adolescent males with HIV having better access to health care."
Murdoch Children's Professor Susan Sawyer said that the COVID-19 pandemic rightly focussed global attention on infectious diseases.
"This study reminds us of the importance of addressing the emerging, as well as the established, infectious diseases, especially in those aged 5-24 years who have too often been neglected by policy and programming," she said.

Regular internet users over age 50 were half as likely to later develop dementia as non-users, a recent study found.
For all its warts, being online might be an overall positive for older people, new research suggests. The study found that reported regular internet use was associated with a reduced risk of dementia in those over 50. The largest possible benefit was seen in people who reported two hours or less of internet use compared to none.
The study was conducted by researchers from New York University. They analyzed data from the Health and Retirement Study, a government-funded project that has tracked the wellbeing of a representative sample of older Americans via surveys conducted every two years. One of the many questions asked of people in the study is whether they use the internet.
The team focused on nearly 18,000 adults over 50 who were dementia-free at the start of the study and were followed for up to 17 years. They then divided the participants into two groups, based on whether they said they regularly used the internet in their initial survey. Another questionnaire assessed people’s cognition, which the researchers used as a measure for dementia status.
The authors found that regular internet users were half as likely to meet the criteria for dementia than those who reported no use at baseline, even after accounting for other factors like their pre-existing health. People who continued to report using the internet in subsequent surveys also appeared to have an even lower risk.
“Taken together, these findings suggest that regularly using the internet may be associated with cognitive longevity,” the authors wrote in their paper, published Wednesday in the Journal of the American Geriatrics Society.
This isn’t the first study to find a correlation between internet use and reduced dementia risk in older people. But the authors say their analysis is the longest of its kind to examine this possible relationship. The Health and Retirement Study is also a prospective study, meaning it proactively tracks people’s outcomes over time. These studies are generally better at rooting out a cause-and-effect link between two factors—in this case, internet use reducing dementia risk—than many other types of research.
There are still many questions remaining about this link, though. The authors also looked at data from a smaller sample of participants who reported how often they used the internet on a weekly basis. They found a possible U-shaped association between hours online and dementia risk, where people who spent the least and the most time online (between 6 to 8 hours a day) seemed to have the highest dementia risk. Meanwhile, the lowest risk was seen in those who spent two or fewer hours on the internet a day. However, the difference in risk for very online people wasn’t statistically significant, perhaps due to the smaller sample size available.
It’s likely that there is a point of diminishing returns, the authors say, where “excessive online engagement may have adverse cognitive effects on older adults.” But more research will be needed to find where this point is. And even if using the internet can help reduce dementia risk, this relationship could cut both ways, the authors note. People who develop dementia might start to use the internet less and less over time, for instance.
At the very least, this study should inspire other researchers to figure out the best balance between staying online and staying mentally sharp, the scientists say.
“Since a person’s online engagement may include a wide range of activities, future research may identify different patterns of internet usage associated with the cognitively healthy lifespan while being mindful of the potential side effects of excessive usage,” they wrote.

ودمج الفريق 8 من هذه المستشعرات في عصابة رأس مرنة ثبتت على الجزء الخلفي من الرأس، وعند دمجها مع مجموعة عصابات رأس الواقع المعزز التي تعرض إشارات بصرية تمكنت الأقطاب الكهربية من كشف الإشارة ثم تعمل مع الكمبيوتر لترجمة الإشارات إلى أوامر تتحكم في حركة الروبوت ذي الأربع أرجل بدون أي استخدام لليد.

vessel stays open longer than it should.
While inside the womb, the lungs are filled with fluid. The baby gets oxygen from the placenta.
Blood flows through the PDA instead of going to the lungs. This blood vessel should close
within a few minutes or days after a baby is born. Then, the blood will flow on its normal path
to the lungs to get oxygen and from the lungs to return oxygen to the heart.
