Source text - English Tertiary peritonitis (TP) is defined as a severe recurrent or persistent intra abdominal infection after apparently successful and adequate surgical source control of secondary peritonitis. However, the term tertiary or recurrent peritonitis may also be used when there is persistent infection after a third reintervention for secondary peritonitis that has been managed with staged or planned relaparotomies. Persistent intra-abdominal infection, an altered microbial flora, failure of the immune response, and progressive organ dysfunction leading to high mortality in Tertiary Peritonitis, ranges between 30 and 64%. The Open Abdomen (OA) procedure may allow early identification and draining of any residual infection, control any persistent source of infection, and remove more effectively infected or cytokine-loaded peritoneal fluid, preventing abdominal compartment syndrome and deferring definitive intervention and anastomosis until the patient is appropriately resuscitated and hemodynamically stable and thus better able to heal.

Meskipun demikian, istilah peritonitis tersier ini dapat juga digunakan untuk infeksi membandel setelah reintervensi ketiga pada suatu peritonitis sekunder yang telah ditangani dengan relaparotomi bertahap atau terencana. Infeksi intra abdomen yang membandel, perubahan flora mikrobiologis, kegagalan respon imun, dan kegagalan organ progresif menyebabkan tingginya mortalitas pada peritonitis tersier ini. Prosedur open abdomen memungkinkan identifikasi dan drainase dini dari sisa infeksi, pengendalian sumber infeksi yang membandel, membuang dengan lebih efektif cairan peritoneal terinfeksi atau yang penuh dengan sitokin, mencegah komparemen sindrom perut, dan menunda operasi definitif dan anastomosis samapai pasien teresusitasi dan hemodinamiknya stabil sehingga mapu sembuh dengan lebih baik.

I am a pediatric surgeon, in Naval Hospital on Tanjungpinang, Indonesia. I started translating scientific papers 16 years ago.
