Study Medication
You will receive study medication at Visit 2, Visit 5, Visit 7 and Visit 8. Note: Your Study Coordinator will fill in the correct visit number in the "Visit __" space on each bottle you receive (see picture below).
It is very important that you follow the dosing instructions below. At each visit, your study coordinator will ask if you took the required number of pills each day. You will also be asked to bring your study medication back to the doctor’s office at Visit 5, Visit 7 and Visit 8 as you will exchange your old bottles for new bottles at those visits.

Rescue Medication - Acetaminophen
You will receive one bottle of acetaminophen from your Study Coordinator at Visit 1. This acetaminophen is to be used on an “as-needed” basis for intolerable Ankylosing Spondylitis (AS) pain that occurs as a result of discontinuing your NSAID. It is very important that you follow the dosing instructions below. Please note that your study coordinator will be asking you how much acetaminophen you took for AS pain at each study visit.
• After Visit 1, Upon Stopping your Previous NSAID (Up to Visit 2)
o You can take up to a total of 4 grams of acetaminophen each day, as needed for intolerable AS pain.

Re-Usable Automatic gun for Biopsy needle
Description:
NEXTAGE is an automatic, reusable, spring-loaded, biopsy gun designed for histological core biopsies. It has an adjustable notch
size of 22 mm or 15 mm.
Intended use: histological biopsy on soft tissues
Important:
• The NEXTAGE biopsy gun is intended to be used with disposable FACILE needles only.
• The FACILE disposable needle, supplied separately, is sterile and non-pyrogenic.
• The FACILE needle is sterilized by ethylene oxide and cannot be resterilized.
• The NEXTAGE biopsy gun is supplied non-sterile.
Precautions:
• The NEXTAGE biopsy gun should be used by expert physicians, who are supposed to be familiar with directions and
contraindications, as well as limitations and possible side effects of core biopsy needles.
• The FACILE needle should be introduced into the patient's body under imaging control (ultrasound, CT, X-ray, fluoroscopy and
so on) for a better needle placement.
• Do not test the NEXTAGE biopsy gun while the needle is inside the instrument, since it may cause damage to the needle itself.
Instructions for use:
NOTE: We do recommend to puncture the skin with a scalpel before inserting the needle.
Use proper anaesthesia prior to any incision of the skin.
A) Preparation of the NEXTAGE:
1. Remove NEXTAGE biopsy gun from its package
2. Load the instrument by pulling the back handles;
Pull the handle 1 to load the cannula (ph 2a); the 'CL' status indicator on the side of the device should turn red(ph.2b) .
3. Pull the handle 2 to load the stylet (ph. 3a); the 'SL' status indicator on the side of the device should turn red too (ph.3b)
Now that both the status indicators are red, the device is loaded and the safety switch automatically on, blocking the fire triggers
until the 'SAFE' button on the side is activated.
4. Test the device: push the 'SAFE' button to remove the safety and press either the front or the back trigger.
5. The instrument is now ready to host the FACILE disposable needle
B) Preparation of the FACILE needle:
1. Aseptically remove the FACILE needle from its package.
2. Remove the protective plastic sheath.
3. Ensure that the needle is free from imperfections.
4. Test the needle by sliding the cannula over the stylet several a few times; there should be no friction.
C) Biopsy procedure with the NEXTAGE:
1. Open the top cover of NEXTAGE biopsy gun.
2. Insert the FACILE needle into the instrument carrier blocks (in case the depth limiting device is fitted, set it at the desired
depth. Make sure that the depth limiting device is positioned outside the Nextage gun) (ph.C1). Close the cover of the
NEXTAGE device(C1a).
3. Load the instrument as indicated in the part “Preparation of the NEXTAGE:”
4. Select the penetration depth (22 mm or 15 mm) rotating the front lever toward the marking of the desired depth.
CAUTION: the depth selection must be done with the device already LOADED. When the gun is not loaded the selection
lever is blocked and any attempt to force it could damage the device.
3. Under imaging control guidance, introduce the needle until its tip has reached the periphery of the area to be sampled.
4. Push the SAFE button on the side of the instrument to release the safety.
THE DEVICE IS NOW READY TO BE FIRED: TAKE CARE NOT TO ACCIDENTALLY PRESS THE TRIGGERS AND INADVERTENTLY FIRE.
5. When ready, press one of the two triggers to fire the gun; this will activate the spring mechanism, pushing the stylet and
then the needle forward.
6. After the firing, remove the needle together with the instruments from the patient's body.
NOTE: IN ORDER TO AVOID TEARINGS OF THE TISSUES, ALWAYS CHECK, BEFORE REMOVING THE DEVICES, THAT NEITHER THE
CANNULA NOR THE STYLET ARE LOADED: THE STATUS INDICATORS ON THE SIDE SHOULD NOT BE RED.
THE NEEDLE DOES NOT NEED TO BE REMOVED FROM THE NEXTAGE INSTRUMENT IN ORDER TO RETRIEVE THE SPECIMEN.
7. To retrieve the specimen, pull the handle 1 to retract the cannula and verify that the 'CL' status indicator is red; remove
the specimen from the needle notch.
8. Load the stylet again by pulling the handle 2.
9. If further biopsies are required, the needle should be carefully inspected to verify that it has not suffered bendings or
damages.
10. If the needle is undamaged, follow again the procedure as described above. If additional biopsies are not required, open
the cover, remove the needle and discard it (see "Limitations and Final Warnings"); press the safety button and fire the
gun to discharge it.
D) Biopsy procedure with the FACILE needle preliminary localization:
1. Insert the needle into the patient's body with the stylet fully retracted, so that the specimen notch is covered.
2. Under imaging control guidance, keep inserting the FACILE needle until the tip has reached the periphery of the area to
be sampled.
3. When the right location is identified, insert the needle into the white plastic carrier blocks of NEXTAGE biopsy gun while
maintaining the correct needle orientation. The device MUST be loaded BEFORE the insertion of the needle.
4. Close the device’s cover, then follow the steps of the above procedure ("Biopsy procedure with the NEXTAGE").
Cleaning:
1. Inspect carefully the device to ensure it does not report any damage or deterioration.
2. After every use, wash the instrument with a mild detergent (e.g. a normal washing up detergent), in warm water: avoid
using any detergent that is not compatible with iron-based metals.
3. Rinse all parts well with hot water.
4. Thoroughly dry the product, both inside and outside.
5. Do not place the device in bleach or bleach solution.
Lubrification:
1. Since cleaning detergents may remove the original lubricant, it is advisable to re-lubricate after the washing.
2. Use a medical grade, teflon- based lubricant after every use (Medax recommend MOLYGUARD FOOD LUBE PTFE H1),
bestowing particular attention to the lubrication of the steel parts that support the needle-holding carriage.
3. Both the detergent and the lubricant should comply with the sterilization cycle utilized.
Sterilization:
1. Sterilize by autoclaving (steam). Do not sterilize by irradiation.
2. Prior to sterilization, the device should be cleaned and placed in appropriate packaging materials.
3. Autoclave (steam) typical conditions are: 250 °F (121 °C +/-2° C) for 15 minutes.
4. For steam sterilization, follow a validated process.
5. In any case, to ensure sterilization effectiveness, the sterilization process itself has to be monitored with the proper
biological controls: any validation of the effectiveness of the sterilization is considered under end user's responsibility.
Limitations and final warnings:
• The use of this device is restricted to qualified physicians only.
MEDAX declines every responsibility for any damage occurred to the patient when the gun is used in an improper way and if
the procedure is carried out with incorrect manoeuvres and/or not by qualified personnel.
• The procedure described is for reference only: the use of the NEXTAGE gun should also be based on the physician's clinical
experience.
• Verify the integrity of the packaging of the FACILE disposable needle and the validity of the product before use.
• After use, carefully remove all device taking all the necessary precautions for potentially biohazardous materials. Dispose of the
products according to the local law.
• During the storing period, please keep the NEXTAGE gun not loaded (the status indicators on the side should not display the
red mark).
• If the NEXTAGE gun has to be sent back to the manufacturer, the user has to clean and sterilize the biopsy system as described
on the previous paragraph before shipping it back.

Warranty - Assistance:
The NEXTAGE biopsy device is covered by warranty for a period of one year from the purchase date. Should any defect (derived
from original parts malfunctioning) occur during this period, MEDAX shall repair or replace the device.
Wear and tear due to normal use or defects resulting from misuse are not covered by this limited warranty.
Medax extend the warranty only if NEXTAGE is used with FACILE disposable needle: in any other case the warranty has to be
considered null and void.

Автоматический пистолет многоразового использования для биопсийных игл
Описание:
NEXTAGE - автоматический, многоразовый, пружинный биопсийный пистолет, предназначенный для гистологической пункционной биопсии. Оснащен регулируемой камерой для биоптата длиной 22 мм или 15 мм.
Предназначение: биопсия мягких тканей для гистологического исследования.
Важно:
- биопсийный пистолет NEXTAGE предназначен для использования только с одноразовыми иглами FACILE.
- поставляемые отдельно одноразовые иглы FACILE являются стерильными и не пирогенными.
- иглы FACILE стерилизованы оксидом этилена и не подлежат повторной стерилизации.
- биопсийный пистолет NEXTAGE поставляется нестерильным.
Меры предосторожности:
Биопсийный пистолет NEXTAGE должен быть использован квалифицированными и опытными врачами, ознакомленными с инструкциями по применению и противопоказаниями, а также ограничениями и возможными побочными эффектами применения игл для пункционной биопсии.
Иглу FACILE необходимо вводить в организм пациента под визуализационным контролем (УЗИ, компьютерная томография, рентген, флюороскопия и т.д.) для лучшей локализации иглы.
Не проверяйте биопсийный пистолет NEXTAGE в то время, когда игла находится внутри инструмента, так как это может привести к повреждению иглы.
Инструкция по применению:
ПРИМЕЧАНИЕ: Рекомендуется сделать надрез кожи скальпелем перед введением иглы.
Используйте подходящий вид анастезии перед выполнением надреза кожи.
А) Подготовка NEXTAGE:
1. Извлеките биопсийный пистолет NEXTAGE из упаковки.
2. Зарядите инструмент, потянув назад за ручки;

4. Испытание устройства: нажмите кнопку “SAFE” для отключения безопасного режима и нажмите на передний или задний курок.
5. Устройство готово к установке одноразовой иглы FACILE.
В) Подготовка иглы FACILE:
1. Асептически извлеките иглу FACILE из упаковки.
2. Удалите защитный пластиковый чехол.
3. Проверьте иглу на наличие дефектов.
4. Проведите испытание иглы, введя стилет в канюлю несколько раз: при этом не должно ощущаться трение.
С) Выполнение биопсии с NEXTAGE:
1. Откройте верхнюю крышку биопсийного пистолета NEXTAGE .
2. Вставьте иглу FACILE в несущий патрон инструмента (если установлен ограничитель глубины, установите его на желаемую глубину. Убедитесь, что глубина ограничителя расположена за пределами пистолета Nextage) (фото.C1). Закройте крышку устройства NEXTAGE (C1a).

7. После приведения устройства в полную готовность, нажмите один из двух курков: будет активирован пружинный механизм, выталкивающий вперед стилет, а затем и иглу.
8. После выстрела извлеките иглу вместе с устройством из тела пациента.

ДЛЯ ИЗВЛЕЧЕНИЯ ОБРАЗЦА НЕ ОБЯЗАТЕЛЬНО ВЫНИМАТЬ ИГЛУ ИЗ УСТРОЙСТВА NEXTAGE.
9. Для извлечения образца потяните за ручку 1, вытягивая канюлю, и убедитесь, что индикатор состояния “CL” красный, извлеките образец из камеры.
10. Снова зарядите стилет, потянув за ручку 2.
11. Если требуется выполнение дополнительной биопсии, тщательно проверьте иглу на предмет ее повреждения или изгиба.
12. Если игла не повреждена, повторите процедуру, как описано выше. Если дополнительная биопсия не требуется, откройте крышку, удалите иглу и утилизируйте ее (см. "Ограничения и Заключительные предупреждения"), нажмите кнопку “SAFE” и разрядите пистолет.
D) Процедура биопсии с предварительной локализацией иглы FACILE:
1. Введите иглу в тело пациента с полностью втянутым стилетом, чтобы камера для биоптата была покрыта.
2. Вводите иглу FACILE под визуальным контролем пока ее кончик не достигнет периферии области, подвергаемой биопсии.
3. Когда достигнута требуемая локализация, вставьте иглу в белый пластиковый несущий патрон биопсийного пистолета NEXTAGE, контролируя правильную ориентацию иглы. Устройство ДОЛЖНО БЫТЬ ЗАРЯЖЕНО ДО введения иглы.
4. Закройте крышку устройства и следуйте описанной выше процедуре ("Выполнение биопсии с NEXTAGE").
Очистка:
1. Внимательно осмотрите устройство на предмет любых повреждений или износа.
2. После каждого использования промывайте инструмент с мягким моющим средством (например, обыкновенным средством для мытья посуды), в теплой воде: избегайте использования моющих средств, несовместимых со сплавами на основе железа.
3. Промойте все части устройства горячей водой.
4. Тщательно высушите устройство как внутри, так и снаружи.
5. Не помещайте устройство в отбеливатель или отбеливающий раствор.
Смазывание:
1. Так как моющие средства могут удалить оригинальную смазку, желательно повторно смазывать устройство после очистки.
2. Применяйте медицинскую смазку на основе тефлона после каждого использования (Medax рекомендует MOLYGUARD FOOD LUBE PTFE H1), уделяя особое внимание смазке стальных деталей, удерживающих патрон для иглы.
3. Использование моющих средств и смазки должны соответствовать используемому циклу стерилизации.
Стерилизация:
1. Стерилизовать автоклавированием (паром). Не стерилизовать облучением.
2. Перед стерилизацией устройство должно быть очищено и помещено в соответствующие упаковочные материалы.
3.Стандартные условия автоклавирования (парового): 250° F (121° C +/-2° C) в течение 15 минут.
4.При стерилизации паром используйте валидированный способ стерилизации.
5. Для обеспечения эффективности стерилизации необходимо проводить мониторинг собственно процесса стерилизации надлежащими биологическими методами: любая проверка эффективности стерилизации подпадает под ответственность пользователя.
Ограничения и заключительные предупреждения:
Данное устройство предназначено для использования только квалифицированным медицинским персоналом. MEDAX не несет никакой ответственности за причиненный пациенту ущерб, если биопсийный пистолет применялся не по назначению, использовался неквалифицированным персоналом или не соблюдалась инструкция по применению.
Описанная выше процедура эксплуатации предназначена только для обязательного исполнения: к использованию устройства должны допускаться только опытные медицинские профессионалы.
Проверяйте целостность упаковки одноразовых игл FACILE и срок годности перед использованием устройства.
По окончанию процедуры утилизируйте все устройства, принимая необходимые меры предосторожности касательно потенциально зараженных материалов. Утилизируйте вместе с прочими медицинскими отходами согласно действующим правилам.
Храните биопсийный пистолет NEXTAGE в разряженном состоянии (индикаторы состояния не должны быть красными).
Перед отправкой биопсийного пистолета NEXTAGE производителю пользователь должен очистить и стерилизовать устройство согласно приведенному выше описанию.

Гарантийные обязательства и сервисная поддержка:
На биопсийный пистолет NEXTAGE распространяется гарантия сроком на один год от даты покупки. Если в этот период обнаружится какой-либо дефект (возникший в результате производственного брака), MEDAX гарантирует ремонт или замену устройства.
Износ в результате нормального использования устройства и дефекты, вызванные неправильной эксплуатацией, не охватываются настоящей ограниченной гарантией.
Гарантия Medax распространяется только в случае использования NEXTAGE с одноразовыми иглами FACILE: в любом другом случае гарантия аннулируется.
