Source text - English • Respiratory disorders are frequently present.
– Both obstructive and restrictive patterns may be seen on pulmonary function tests.
– Reduced respiratory muscle strength, impaired cough, and sleep apnea are common.
• Asymptomatic cardiac disease may be present and undiagnosed due to the patient’s
limited mobility and dementia.
– Levodopa increases heart rate, cardiac contractility, and is associated with both
hypertension and hypotension.
• Oropharyngeal dysphagia and esophageal dysmotility may predispose to aspiration and may impair nutrition.
• Autonomic dysfunction is present in 15–20% of patients and may predispose patients to orthostatic hypotension, constipation and ileus, and voiding dysfunction associated with urinary tract infections.
• Dementia is seen in one-third of patients.
• Psychosis, including hallucinations, is common and is related to dopaminergic and anticholinergic therapies.
• Older age and immobility increase risk for venous thromboembolism and decubitus ulcers. | Translation - Russian • У пациентов часто присутствуют респираторные расстройства.
– Измерение легочных объемов показывает расстройства обструктивного и рестриктивного типов.
– Часто наблюдаются ослабление дыхательных мышц и кашлевого рефлекса, а также апнэ во сне.
• У пациента могут присутствовать бессимптомные кардиологические заболевания, не диагностированные вследствие его ограниченной подвижности и деменции.
– При приеме леводопы усиливается сокращаемость сердца и учащается сердечный ритм. С действием леводопы одновременно связывают развитие гипертензии и гипотензии.
• Орофарингеальная дисфагия и пищеводная дискинезия могут привести к аспирации и затруднить питание.
• Автономная дисфункция присутствует у 15-20% и может привести к ортостатической гипотензии, запору и непроходимости кишечника, дисфункции мочевого пузыря на фоне инфекции мочевыводящих путей.
• Деменция наблюдается у 1/3 пациентов.
• Психоз, в т. ч. с галлюцинациями, — распространенный побочный эффект, обусловленный действием допаминергических и антихолинергических препаратов.
• В силу пожилого возраста и неподвижности повышается риск ВТЭ и образования пролежней.
