Tumor vaccines in renal cell carcinoma
Introduction
It is well known that renal cell carcinoma (RCC) has unique biological behaviors, for example, spontaneous tumor regression and long dormancy in metastatic lesions is occasionally observed [1]. Investigations regarding such evidences suggest that the host immune system plays an important role for tumor rejecting cell-mediated immune responses. Therefore, immunotherapy, active and passive, as well as specific and non-specific, seems to be a suitable therapeutic alternative for RCC. Recently developed molecular targeting agents particularly anti-angiogenesis agents, such as sunitinib and sorafenib, have been proven worldwide as first and second line standard treatments for metastatic RCC. Despite their significantly powerful anti-tumor effects, most of the patients with metastatic disease cannot be cured. To date, combination therapy as well as sequential therapy of sorafenib or sunitinib with interferon- alpha is under investigation. Because of completely different anti-tumor mechanism, clearly, immunotherapy still remains a basis of promising treatment strategies for metastatic RCC.
Autologous tumor cell vaccines
Historically, cell-based vaccines were comprised of nonviable autologous tumor cells or some form of preparation that would alone provide a source of antigens necessary to elicit an immune response. This is based on the notion that RCCs themselves express TAAs that will in turn initiate a cytotoxic T cell response. This system has the potential advantage in that unknown TAAs are presented, however it was determined early that stronger immune response is necessitated for a strong therapeutic eVect. Thus, the use of traditional adjuvants like incomplete Freund’s adjuvant, alum, QS-21, as well as newer molecules such as IL-2, IL-12, granulocytemacrophage colony stimulated factor (GMCSF), bacillus Calmette–Guerin (BCG), and highly purified heat shock proteins have also been employed to enhance immune responses.
Future of RCC vaccine therapy
As technology evolves, the promise for the development of strategies aimed at circumventing tumor-associated immunosupression will certainly improve the efficacy of tumor vaccines. Furthermore, as new TAAs or immunostimulatory peptides (endogenous or synthetic) are discovered, the prospect of personalized cancer therapy becomes more imminent especially with peptide-based tumor vaccines. Therapeutic vaccine therapies have been limited to advanced or metastatic disease; future studies should explore their effectiveness in cases where disease is not as advanced and an individual’s immune system is not compromised.
Finally, the efficacy of combination therapy of tumor vaccines with chemotherapeutic agents needs to be explored.

Противоопухолевые вакцины в почечно-клеточной карциноме

Известно, что почечно-клеточная карцинома (ПКК) обладает уникальными биологическими свойствами, например, иногда наблюдается спонтанная регрессия опухоли и длительная латентность в метастатических очагах [1]. Исследования, связанные с данными свойствами, предполагают, что иммунная система организма-носителя играет важную роль в клеточных иммунных реакциях отторжения опухоли. Поэтому активная и пассивная иммунотерапия, так же как специфическая и неспецифическая, представляется подходящей альтернативой лечения почечно-клеточной карциномы. Недавно разработанные молекулярно-таргентные препараты, в частности, антиангиогеннные препараты, такие как Сунитиниб и Сорафениб, получили всемирное признание как стандартные препараты первой и второй линии для лечения метастатической почечно-клеточной карциномы. Несмотря на их значительно сильный противоопухолевый эффект, большинство пациентов с метастатической болезнью не поддается лечению. В настоящее время исследуется как комбинированная терапия, так и последовательная терапия Сорафенибом или Сунитинибом с интерфероном-альфа. В связи с совершенно различными противоопухолевыми механизмами, очевидно, иммунотерапия по-прежнему остается основой для многообещающих стратегий лечения метастатической почечно-клеточной карциномы.

Эта система имеет потенциальное преимущество, в котором представлены неизвестные антигены, однако вскоре было определено, что для сильного терапевтического эффекта требуется более сильный иммунный ответ. Таким образом, для увеличения иммунного ответа использовались традиционные вспомогательные факторы, такие как неполный адъювант Фрейнда, квасцы, QS-21, а также были задействованы более новые молекулы, такие как IL-2, IL-12, гранулоцит-макрофаг-колониестимулирующий фактор (GMCSF), бацилла Кальмета-Герена (БЦЖ), и высокоочищенные белки теплового шока.

Наконец, должна быть исследована эффективность комбинированной терапии противоопухолевыми вакцинами с химиотерапевтическими средствами.

Later that same year, I sat and passed a TOEFL exam, and shortly afterwards I embarked on a new career as an interpreter and translator.
Since 2005, I have translated many business documents using the languages of which I have an expert command, i.e., Russian, Ukrainian, and English. In addition to translating manuals, websites, and contracts, I have also taken part in international negotiations as an interpreter and company representative.
