It could also be this format: MÉDICO/A CIRUJANO GENERAL. If you copy and paste this text in quotation marks in google you will see a number of webpages using the exact phrase.
Aquí, seguido de "general", no parece tener mucho sentido lo de atención continuada. Aunque ni idea; sin otro contexto, y asumiendo la idea de cirugía general, médico ambulatorio, médico asistente, o incluso Manolo Arias, cirugía general.
