Hi David,
It would be "history by system(s)", but "by system" is just not used in the US; not sure about the UK/Australia, etc. The "system" bit is sometimes used, however, to refer to the physical exam (see example below).
From your text, I'd skip it...

por aparatos, i.e. aparato respiratorio, circulatorio, digestivo, etc.
I think that "per system" would be a good choice for this one.
