This is from a series of information fields at the top of a hospital admitting form, from Spain. Some of the other fields are:

Servicio Peticionario
On the row where this term appears we have:
N. Cama // F. Nac.-SX // ALTA CON/HOS = SÍ
This seems to have to do with the discharge, but I can't figure out "con/hos". And, this patient wasn't really 'discharged' but passed away in the hospital. I appreciate your help with this mysterious abbreviation!

That being the case, I'm slightly closer ... ¿but how would we translate Alta Consulta / Alta Hospitalaria?

I actually assumed that CON stood for "consultorio" in my translation, but I'm not completely sure of it... so I'll say that it might also have been "consulta". We'll let the next person who consults this term choose the option they think is best."
