Well, when I interpret at the local job centre they usually ask for a "contact number" or "home telephone number"

Hi,
I have seen these terms used more often in forms:
- home phone: instead of home telephone number. Apparently, it is too long.
- number where you can be reached: I have never seen "contacted", only reached.
