показав белые с желтоватым отливом зубы, она улыбнулась.

A guy describes a girl who is lying in bed beside him as he wakes up after partying all night. I understand what he is saying but how to put it in decent English!

Or, if you think that's a bit overstated, you could go with "slightly yellowed teeth"

agree The Misha : My own first impulse was somewhat/slightly yellowish teeth, and that's probably how most natives would think of it. The problem is we are not writing this text, so I guess, we gotta give them white teeth with a yellowish tinge or something like it.

pearly white smile

In the described setting, the use of "yellow" would rob the narrative of its (it is supposed it is) romantic tone.

A possible variant is " she gave him a pearly white smile" or " she smiled revealing her pearly whites.

Pearly whites are supposed to be yellow in any case.

off-white teeth

off-white = a grayish or yellowish white.

Example
