Regarding the "registry" Perhaps it is a program for people with specific chronic conditions? Something like this: http://www.monashhealth.org/page/Chronic_Disease_Management_... but for people with chronic kidney failure.

More context please Can you provide context on the patient's disease/medical conditions that would help identify ХНИ? If you have the entire discharge summary, you should be able to get some more clues.

Not sure Kimberly. It just might be an option to receive care in more specilized settings or an opportunity to receive required teratment.

Does it mean that the Nephrologist must be a registered provider for CKF treatment? You know like preferred provider list for insurance companies?
