intense and heterogenous contrast with the use of the contrast agent/medium
Perhaps with more context a different suggestion could be made, but as it stands I believe the sentence refers to something that could be observed with the administration of the contrast agent

intense, heterogeneous image/die contrast enhancement
There is a big difference between the two forms of enhacement:
"Homogeneous enhancement is suggestive of a benign process, however, again, in small lesions, one must be careful as spatial resolution may limit evaluation. Heterogeneous enhancement is more characteristic of malignant lesions especially if rim-enhancement is present." - https://www.cambridge.org/core/journals/breast-cancer-online...
Basically, contrast enhancement occurs in two ways:
- by administering contrast media/agents, and
- in image post-processing.
See https://radiopaedia.org/articles/contrast-enhancement?lang=u...
The context is insufficient to draw a safe conclusion; therefore I included both options separated by a slash. I strongly suggest consulting the client, or simply leave out "image/die" from the translation.
Example Sentences
"MIP image (A) of the enlarged right breast in a 47-year-old woman displays intense heterogeneous contrast enhancement and vascularity." - https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4557414
"Post-gadolinium spin-echo T1-weighted axial image shows the intense heterogeneous contrast enhancement of the extra-axial mass with thickened falx cerebri (arrow)." - https://www.eurorad.org/case/1752
