I know that 国保 is the Japanese national health insurance, but I am not sure how to translate 高規給. High grade?

Hi conejo, やはり気になったので、I just called a city office's insurance department and they replied that 高規給 is most probably a 変換 typo for 後期、 which is short for 後期高齢者 (elderly over 75 y.o., note that this doesn't indicate all elderly). Probably this could be circled by hospital if a person was elderly over 75 yo、or just left blank if the patient is younger. They said they could not think of any other こうき、while 給 is usually used for indicating 割合負担、most likely elderly only bear 10 percent of the amount. Anyway, if you could confirm with your client or make a call to any insurance department, they might clarify more if case you have more details.

If it is a hospital receipt, could it contain information about benefits for medical care? Not sure. I will try to give a call to my 区役所の保険課、and will let you know on Monday what they say. Cheers!

If the meaning is 後期高齢者、there could be a typo or 変換ミス 、but if, as you said, the person is not elderly, then it is not easy to guess.
and 給、is it 給付 or 給与, nobody can tell for sure. Regards, Tokyo_M

Hello Conejo, thank you for your reply. Well, I just went to local 整骨院 where I undergo treatment, and asked the manager about this strange abbreviation. He also said, that the person could be 後期高齢者 (person above 75 years old) who receives 給付,
but if the person is not eldery, it could very well be (probably not 給料)、but 給与 in the contest of calculating insurance fee based on income, see the following:

Surrounding data The surrounding data did not have any information about the person's income. It was just information about the person's illness, and how much each medical care item cost.

Context It was a line on a hospital statement about what kind of insurance the person had. There was no more information about the insurance. The person was not elderly, so I don't think 「高」 is 高齢者. I understand Tokyo Moscow's statement about more context being good, and I agree wholeheartedly, but there wasn't any more context; that's all I have. Thanks everyone.

Hello Conejo, could you please give more contest? It is very difficult to guess without that, as you know, it could mean just anything and gives people right to disagree based just on pure guesses. Let's assume that apart of any contest 国保 is a national health insurance indeed, but next term could perfectly be not about 保険, but about person's income, etc. If there was more contest in the beginning, translator with experience could probably guess better, that just giving (wrong? and unreasonable) grading to others)))

高度医療の規定による給付? 国民保険制度は世帯ごとの所得に対して計算されるものなので、「給与」とは関連しないと思います。
この略語を目にしたことはないのですが、個別の病院などによる略語なのでしょうか?
構造改革特区により、高度医療を行う施設に規定により給付金が出るので、それに関連する項目かもしれません。

Character 給 here is for 給料 , ie salary,
so 高規給 should be "high-salary" or "high salaried".
Could be also "high income" (jobs, employees, etc)

Also it could be a typo for 後期 (short for 後期高齢者), that indicates elderly over 75 yo.

This is just a wild guess, but it could be an abbreviation of
「高額療養費」補助制度の規定による給付
"benefit based on the provision on high-cost medical care"
