This seems to be a relatively new guideline. All I have in the way of context is the title of an academic workshop in the field of nursing:

"Haftungsrechtliche Situation im modernen Wundmanagement unter Einbeziehung der für das Wundmanagement relevanten Aspekte der Heilkundeübertragungsrichtlinie."

Übertragen in this context means more than just delivering or applying medical treatment, it means transferring/delegating medical tasks to nurses, i.e. authorising/empowering them to perform a number of tasks that carry added responibility

I would tip that you keep the German term with an explanation in brackets or as a footnote. Two possible renderings that come to my mind, based on my reading of the reference I've posted separately:

or
Directive on the devolution of medical tasks and duties to (qualified) nursing care and geriatric care professionals

health delivery guideline

The word is referring in general terms to the formal title of the relevant guidelines, and is not the name of a guideline.

For that reason I would be tempted to use it in the plural, "health delivery guidelines".
