venular pO2 as a measure of oxygen extraction
That's basically just an interpretation: Oxygen extraction (from the blood into the cells) is a function of microcirculation. It is measured by estimating the drop in pO2 occurring in the capillary bed. In a person with healthy lungs, pO2 on the arterial side of the circulation (down to the arterioles) can be approximated with a constant value; hence, pO2 in the venules will yield an estimate of oxygen extraction, viz of microperfusion.
