Not sure about the term Substitutionspflichtigkeit. Could the sentence be rewritten as "After 10 days, treatment had to be discontinued due to a drop in thromboplastin time and increase in partial thromboplastin time requiring replacement with fresh plasma for 2 weeks" or is there an actual set term for this? Thanks!

I think David's "replacement therapy" is basically fine, but I wouldn't use it in this particular sentence of typical German medicalese. In a sentence about "replacement with fresh plasma" I would normally expect information on WHAT is being replaced. After all, it isn't "the drop in thromboplastin time and increase in partial thromboplastin time" that is being replaced, but it does make topping up with fresh plasma necessary.

plasma substitution

I think you can retain "substitution" in the English, it conveys both of the necessary ideas of a substance having been lost and then needing replacing.
