The question I have is: Can this refer to just an erroneous transaction or must it involve some type of dishonest practices? I know in the dictionaries, it says "forged," "counterfeit" and the like.

Fälschen can't mean erroneous; it means to falsify. I think you ought to consider buying a monolingual German dictionary, such as the Duden Universalwörterbuch or Wahrig.
