This is a pharmacovigilance email and I apologize for all the redacted sections:
"la TEC EXTERNE a reporté dans l'eCRF ces données xxx dans les pages correspondantes xxx en PJ sans pouvoir saisir nominativement, dans chaque dernière page "xxx" de ces 12 cycles xx, les informations d'EIs rapportés par le médecin xx"
What would "saisir nominativement" mean in this context?

It's hard to be sure without knowing the full circumsatcnes and why this is being said, but it seems to me that although Dr X has reported a number of adverse events, it was not possible for them to be linked to specific (named) patients — which, for example, have permitted further follow-up work to be done.
'nominatif' usually refers to named / identified information, as distinct from anonymous. What we don't know here, of course, is exactly who it is who were not able to be named (nor why).

saisir = enter (details, etc.) — in a field on a computer or on a paper form
