: I personally wouldn't go for 'sheet' (unless it actually is a separate, loose-leaf sheet), but I like TR's suggestion of 'log', and your own 'record' is OK too.

Document development chart/table
This refers not to the history of the document , but its preparation/development giving details of all details regarding the document, giving the revisions. Sometimes the reasons for the revisions made are also mentioned.
