when nipples are visible through clothing

Evidently, this is a term frequently used in reference to nipples showing through clothing - google it ~ it is surprising.

I have no idea what 'nipple pokey in hospital' could mean ~ HTH

breastfeeding-----sucking or poking, it depends.

It all depends.
If the context is talking about a woman and her newborn baby, then "pokey" here means the action of the baby sucking the nipple, as he/she feeds on the nipple, he/she will "poke" at it.
If the context is just talking about a woman, maybe one just before giving birth to a baby, then "pokey" means her nipple is becoming stiff, maybe the milk is growing very well and ready to feed her soon-to-come baby.
If the context is just talking about woman, not pregnant, then "Pokey" means she is stacked and her nipples "poke" at her clothes and easily observable.
For the first meaning above, please refer to the link provided; for other meanings, just google it and you'll get it.

agree Demi Ebrite : I do hope David will check back in here - I was unsuccessful at finding the use of the word 'pokey' with regard to breastfeeding, which it must be, if the hospital reference is literal. I am glad you found this!
