A pellet die is a wheel with little holes, used in many different sectors for differnt things, like animal food, etc.
For ophtalmology purposes it is used to make inserts.
The first link shows the machine being used to make animal food, and names the part you listed as the extrusora. The second one shows all the different part of the extrusora.
