Boca arriba para dormir, boca abajo para jugar

This is a translation from patient/family information for Children's Hospital of MN. I already know that the translation is Back to sleep, tummy to play, but if anyone can give me a general explanation as to why we use the terms "boca arriva" and "boca abajo" as a translation for "back" and "tummy" that would be great. Why cant we use "espalda para dormir, barriga para jugar"?
