Устоявшийся перевод мне не известен. Скорее всего, его пока вообще нет, т.к. этот уставный документ, вроде бы, был введен относительно недавно (вместо меморандума). Так что пока имеет смысл сопровождать перевод оригинальным наименованием в скобках:

Описание этого документа:
Notice of Articles: This is a prescribed form that must be filed electronically. It must show the name of the corporation that has been reserved. If you intend to incorporate as a "numbered company", the name of the corporation would be left blank. The notice of articles must also show the full name of each director together with the mailing and delivery address for each. The registered and records offices for the corporation must be stated. The share capital for the corporation must also be set out in the Notice of Articles. If there are any special rights and restrictions attached to the shares, this must also be shown in the Notice of Articles.

Хотел бы подчеркнуть, что Notice of Articles - НЕ ЗАЯВКА и НЕ ЗАЯВЛЕНИЕ. Этот документ прилагается к заявке.
Скорее уж его можно было бы назвать "Краткие сведения об учреждаемой компании" или "Справкой об учреждаемой компании".

О том, что это два разных документа, свидетельствует хотя бы раздельная нумерация страниц, т.е. там не один документ (заявка) размером в четыре страницы, а два документа по две страницы. Ну, это так, дедукция и логика. Если же ссылаться на мнение самих канадцев, то приведу и его:
Under the new Act, BC Companies will have to file an Incorporation Application AND (выделено мною) a Notice of Articles (in the old Act, this was the memorandum).

An incorporation application referred to in subsection (1) (b) must

(a) be in the form established by the registrar,
(b) contain a completing party statement referred to in section 15,
(c) set out the full names and mailing addresses of the incorporators,
(d) set out (i) the name reserved for the company under section 22, and the reservation number given for it, or (ii) if a name is not reserved, a statement that the name by which the company is to be incorporated is the name created by adding "B.C. Ltd." after the incorporation number of the company, AND (ВЫДЕЛЕНО МНОЮ)
