Waiver of 30-Day Waiting Period

Internal Revenue COde requires a plan sponsor toprovide each participant, within 30 to 180 days before teh annuity strating date, a written explanation of a partcipant'sright, and the effect of, an election to waive the joint and survivor annuity form of benefit, if married and the life annuit form,if single.

X cannot gebin your benefit payments while you are still covered under the 30-day minimum election period.Your plan allows you, by signing this form,to waive this election period from the minimum 30 days to the minimum of 7 days.
